Description MaineHealth Geriatric Medicine is seeking a Nurse Practitioner (NP) or Physician Associate (PA) to care for patients in the hospital at MaineHealth Franklin Hospital in Farmington, Maine. Nurse Practitioner or Physician Associate will be responsible for providing primary care services, including assessing, diagnosing, prescribing, treating and educating patients. Works closely with physicians and other clinicians in a team approach to patient care. This person will work in the nursing home setting providing care to post-acute and long-term care patients. The PA or NP will work with a team of geriatricians and other APPs in a shared patient model. Description MaineHealth Primary Care, Internal Medicine is seeking an Experienced Full-Time Physician Associate (PA) or Nurse Practitioner (NP) to provide care at ouroffice based MaineHealth Primary Care, Internal Medicine Practice located in Farmington, Maine. The successful candidate will join our dynamic team including four physicians and three Advanced Practice Providers (APP), working together with outstanding support staff. MaineHealth Internal Medicine services provide office based primary care medicine services and preventive and diagnostic cardiovascular services and serve a population of approximately 40,000. The local medical community provides excellent collegial interaction and referral relationships.
