Set primarily on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, Gurwin's Assisted Living community (Gurwin Jewish ~ Fay J. Lindner Residences), our independent living community (Fountaingate Gardens), Island Nursing and Rehab Center, and two certified (CHHA) and licensed home care (LHCSA) agencies. POSITION : part-time all shifts 7:00am-3:15pm, 3:00pm-11:15pm and 11:00pm-7:15am (scheduled every other weekend) Requirements include : •Graduate of an accredited professional nursing program with a current license to practice in New York State. •Demonstrates interest in the special needs of geriatric patients and possesses an ability to integrate principles of geriatric nursing into nursing practice. •Ability to establish effective interpersonal relations with patients, staff and families. •Ability to communicate effectively in English, both orally and in writing. •Education in all areas of LTC/Subacute to ensure competency on all units in the facility. Responsibilities include, but are not limited to : •Participates in the nursing care of patients within the scope of the legal definition of the practice of practical nursing. The LPN works under the supervision and/or direction of a registered professional nurse. •Participates in the formulation and implementation of patient care plans to meet the specialized needs of geriatric patients under the supervision of an RN. •Administers patient care as assigned by an RN within departmental guidelines. As the unit situation requires, an LPN may be assigned to assume team leader, primary nurse or charge responsibilities under the direction of an RN •Directs and supervises non-professional staff as assigned. •Observes, records and reports patient data as dictated by departmental policy. •Participates in unit nursing and inter-disciplinary team activities. •Administers medications and performs nursing procedures as dictated by departmental policy. •Attends nursing and other institutional in-service programs and participates in departmental committees as assigned. •Performs related and miscellaneous duties as directed. Evaluates care provided for patient outcome: Demonstrates ability to measure effectiveness of care provided and documents same; Performs variance analysis related to outcome data for performance improvement; Designs, implements, and evaluates systems to improve care in Unit; Keeps accurate documentation of patient’s treatment, activity and condition; Uses appropriate methods of documentation according to departmental policy. Administers medications correctly and safely: Correctly identifies medication (action; dosage; side effects and implications for the patient); Meets standard on medication administration examination; Demonstrates preparation of local solutions; Demonstrates preparation of I.V. drips; Administers and documents medication correctly; Administers IVP medication as per department policy and procedure; Educates patients and significant others related to drug and food interactions. Participates in patient and family education: Provides the patient with an explanation of his/her condition as indicated; Communicates assessment data in an orderly fashion by recording, updating and verbalizing pertinent information to care team members and to appropriate agencies; Recognizes and utilizes health teaching opportunities and resources /materials available for this teaching; Provides for early discharge planning and appropriate referrals for post-procedural or operative care; coordinates appropriate referrals for follow-up care, specialty services, or community resources; Evaluates the effectiveness of teaching by feedback from patient/family and documents same. Must be enrolled in an accredited BSN program within two (2) years and obtain a BSN Degree within five (5) years of job entry date. Current license to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed. Previous Experience In Labor And Delivery/Antepartum Highly Preferred. Rounds the nursing units daily to ascertain the condition and needs of the residents and informs Director of Nursing of all changes and related problems. Observes nursing care of the residents to ensure that orders are carried out as directed and treatment is administered in accordance with physician's instructions. Assists with orientating newly employed nurses and clinical placement of nursing students. Arranges hospital transfer of residents. May render nursing care, administer medication and perform treatments. Assists in planning, developing, organizing and implementing departmental goals and objectives. Ensure appropriate staffing levels are met for each unit by coordinating with Staffing Coordinator Exhibits creativity and initiative in pursuing organizational and departmental growth and development.
