Job Summary The Hospice PRN Registered Nurse provides skilled nursing care hospice following physicians’ orders and the established Plan of Care. This position participates in the supervision of Home Health Aides and LPN's. Coordinates and collaborates with the interdisciplinary team ensuring the enactment of the Plan of Care. Reports to the Clinical Nurse Manager. Works and schedules a minimum of assigned shifts and call shifts according to PRN contract commitment. Informs physician/provider and other team members of changes in the patient’s needs and outcomes of intervention, while evaluating patient and family response to care. All documentation is submitted in a timely manner and per documentation policy. Communicates patient information as necessary to the interdisciplinary team and other nurses as required. (2) EMT or above certification. Candidate will complete the UVA PCT Course within 6 month probationary period (3) Graduate of an accredited nursing program. PHYSICAL DEMANDS Job requires frequently bending/stooping and driving (Home Health Aide only). Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 – 50lbs and > 100lbs (Home Health Aide only). May be exposed to chemicals, blood/body fluids and infectious disease The starting base rate for this role is $15.37 hourly. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
