Job Description: Essential Functions: Provides nursing care using a Family Centered approach. Assesses individualized patient needs. Communicates patient care needs and priorities to assigned Licensed Practical Nurses (LPNs), Patient Care Assistants (PCAs), Unit Coordinators, and Allied Health personnel. Plans, implements, educates and evaluates care using Evidence-Based Practices. Utilizes Therapeutic Communication while monitoring the physical, psychological, and social needs of patients and families. Engages in the “workplace community” by staying aware of changes, participating in data collection or auditing, understanding Quality Improvement, and representing staff at relevant meetings and then reporting important information and updates back to staff. Documents according to organizational standards, policies, and procedures. Education Requirement: BSN or Associate degree with commitment to obtain BSN within 5 years of hire, required. Licensure Requirement: Licensed as a Registered Nurse in the State of Ohio. Certifications: Current CPR certification required. Must be willing to obtain and maintain NRP, PALS, ACLS, APHON or other necessary certification as determined by your manager based on business unit. JOB LIMITATIONS The hospice aide will not function in any manner viewed as the practice of nursing according to the Nurse Practice Act. Specifically, the hospice aide will not administer medications, take physician's orders or perform procedures requiring the training, knowledge and skill of a licensed nurse, such as sterile techniques. POSITION QUALIFICATIONS 1. Completion of a state approved certified nursing assistant course or equivalent training 2. Current Healthcare Provider CPR (Cardiopulmonary Resuscitation) certification ((may not be internet based unless the demonstration of skills was hands on and observed by a certified trainer) 3. Due to nature of job and working with multiples facilities with multiple covid guidelines must be covid vaccinated. 4. High school diploma or General Equivalency Degree (GED) 5. Ability to read and follow written instructions and document care given. 6. Understands hospice philosophy, comfortable providing specialized care to the terminally ill. 7. Prior experience in hospice, orderly, acute , rehabilitative, or long term care facility. 1 year previous home health/hospice experience preferred 8. Basic computer skills to document in electronic medical records (EMR) 9. At least 18 years of age. 10. Licensed driver with automobile that is insured in accordance with organization requirements and is in good working order.
