Overview Ensures coordination and continuity of patient care for heart transplant and heart failure patients in order to maintain organized and efficient care and provide support to staff and other health care professionals. Minimum of three years experience in nursing, preferably with experience in critical care, transplantation, or acute care experience with the relevant patient population (i.e. heart, liver, VAD, lung, kidney). For pediatric positions, 1 year of related experience must be in the pediatric setting: eligible for DEA registration.
