Re-evaluates client’s nursing needs and reports findings to Case Manager. Counsels and instructs the client and family in meeting nursing and related needs. Prepares clinical and progress notes. Submits all documentation in a timely manner in accordance with agency policy. Documentation on patients chart correlates with nursing care plan. Communicates frequently and works closely with other home care team members. Our Certified Nursing Assistant – CNA (State Tested Nursing Assistant-STNA in Ohio) work with community management to provide seniors with personalized care, and give resident status updates at the beginning and end of each shift.
