Minimum Education Required High School Diploma/GED Shift First Shift Responsibilities Assist residents with all aspects of medical management including physicals, referrals, prescriptions, appointments and treatments. Provide emergency first aid in the case of a medical crisis. Coordinate with residents, family members, physicians, other medical providers, and site team regarding all medical care and changes in status, treatment or medication. Implement, oversee, document and supervise medication management in accordance with agency and funder regulations. Oversee direct care staff, under the direction of the Residential Program Nurse. Monitor and evaluate the continuity and level of care provided, devising ideas for quality improvement when needed. Participate in staff meetings, trainings, service planning meetings, resident and family meetings. Qualifications Massachusetts as a Licensed Practical Nurse and 1 year experience in a community healthcare setting. Strong oral and written communication skills with individuals, families, funding sources, consultants, the community at large, and the program and administrative staff. Excellent written and verbal communication. Must hold a valid driver's license, have access to an operational and insured vehicle and be willing to use it to transport clients. Strongly prefer a candidate that will have a demonstrated understanding of and competence in serving diverse populations. Assesses patient pain level upon admission and as necessary, depending upon diagnosis utilizing the numerical pain scale. Reassesses effectiveness of pain control and notifies physicians as needed, as well as, documents assessment and patients’ response to medication. Performs and documents initial assessment upon patients’ arrival and implements triage protocols as needed. Identifies patients at risk for potential/alleged abuse. Initiates safety plans for domestic violence, child, and elder abuse patients. Completes required mandated reporting in a timely manner. Collects and documents forensic evidence as per policy. Maintains chain of evidence per policy and procedures Completes mandated reports for stabbings, gunshot wounds, burns, and sexual assault per policy and procedure. Maintains knowledge of Disaster Policy and Procedure. Identifies patient at risk for violent behavior and appropriately implements non-violent intervention (i.e. Code 100). Demonstrates knowledge of the indications for, alternatives, monitoring and documentation of restraint to use. Recognizes normal and abnormal laboratory and diagnostic results and appropriately notifies the physician of changes in a timely manner. Safely administers vasoactive medications demonstrating understanding of dosage, action, use, side effects, and titration parameters associated with maintenance of critical drips. Performs accurate and timely identification of cardiac arrhythmias. Institutes, monitors, and documents thrombolytic and anticoagulation therapy standards and protocols. Demonstrates skills associated with initiation and maintenance of the intubated patient including, suctioning, troubleshooting, and weaning. Accurately interprets arterial blood gases, pulse oximetry, and clinical features of impaired gas exchange and/or airway clearance. Responsible for obtaining supporting information and completing required documentation related to medication reconciliation for use by the Attending MD. Responsible for implementing components of the VAP bundle consistently and per protocol. Responsible for alerting the Nurse Manager/Director/Nursing Supervisor if there is a deviation from VAP bundle by Attending MD for case review and follow up as indication. Maintains sterile fields and directs the correct care, handling, and sterilization of sterile supplies and instruments. Identifies patients correctly by checking patient identification bands and consent forms as well as verifying operative procedure and site before taking patients to the operating room. Applies principles of aseptic practice, oversees the selection and preparation of supplies, equipment and instrumentation to all surgical cases. Applies principles of body mechanics and adapts them to various situations during positioning and transfer of patients. Assures that all techniques and practices carried out during the surgical procedure are accurate and reflect standards of perioperative practice. Maintains sponge, needle and instrument counts according to policies and procedures, including notifying the surgeon of the result of the counts. Maintains neat and accurate narcotic control inventories and all documentation related to the administration of medications. Promotes patient's independence by establishing patient care goals, teaching patient to understand condition, medications, and self-care requirements, answers patient questions. Assures quality of care by adhering to therapeutic standards; measuring health outcomes against patient care goals and standards; making or recommending necessary adjustments; following philosophies and standards of care set by State Board of Nursing, and other governing agency regulations. Resolves patient problems and needs by utilizing multidisciplinary team strategies, including de-escalation techniques. Documents patient care services by charting in EMR. Maintains continuity among teams by documenting and communicating actions, irregularities, and continuing needs. Contributes to team effort by accomplishing related results as needed. Participates in the onboarding of new nurses for medication administration. At end of shift communicate to incoming LPN pertinent data regarding patient activity and conducts medication count. Treats patients, and colleagues with dignity and respect; honors each person’s right to privacy/maintains very strict patient and colleague confidentiality; respects all individual differences; supports cultural diversity. Knows role and responds appropriately to all codes related to emergency situations. Consistently arrives to work on time and adheres to attendance policies. Other duties, as needed.
