Position Summary: We have an exciting opportunity to join our team as a Peds Acute Care NP or PA in the PICU in Manhattan. In this role, the successful candidate will be responsible for the provision, coordination, and assurance of comprehensive nursing care using nursing process and pertinent standards of care, the diagnosis of illness and physical conditions, and performance of therapeutic, corrective, and prescriptive measures of the designated inpatient or outpatient patient population within a collaborative practice agreement established with a specific physician on the staff of NYU Hospitals Center Job Responsibilities: Plans and organizes care to meet individual patient needs and to ensure appropriate clinical resource utilization per protocols, pathways, and other means. Monitors and trends the cost effectiveness of the position, particularly as it relates to the efficiency and performance indicators established for the service. Assists in the preparation, implementation, and evaluation of research protocols when applicable. Works collaboratively with nursing and other disciplines in the development and implementation of clinical studies within the area of expertise. Uses evidence-based health care literature to advise and support appropriate practice changes within the designated service. Participates in the development and monitoring of patient outcomes per established practice protocols for purposes of quality and performance improvement. Serves as a clinical expert and resource for the education of peers and other health professionals. Maintains current expertise in area of practice. Promotes own professional growth and development in clinical and managerial role. Considers needs and behaviors of specific patient age and cultural groups in all patient care. Evaluates treatment and health care plans for effectiveness and modifies per clinical standards and practice protocols. Practices as a member of the allied health staff according to the rules and regulations of the Medical Staff and bylaws as outlined in the delineation of privileges. Prescribes medical equipment, devices, physical and occupational therapy, and home health services per practice protocol. Prescribes in-patient and discharge medications according to New York State law, including controlled substances with DEA authorization. Works collaboratively with the attending physician, consulting physician(s), and other disciplines to identify, develop, and implement an appropriate plan of care that maximizes individual patient/family preference and enhances quality, access, and cost-effective outcomes. Formulates the plan of care, along with the attending physician, patient and family, based on expected goals of care and length of stay. Orders customary laboratory, radiological and diagnostic studies per practice protocols, differentiates between normal/abnormal findings and follows protocols for abnormal findings, which may include referral to consulting physicians per customary pattern of the attending MD or with the collaborating physician. Performs complete health history, physical examination and psychosocial assessment of patients within the designated practice or clinical unit. Functions as a respectful member of the health care team. Participates in designated activities related to professional nursing standards and regulations of relevant outside agencies. Demonstrates knowledge of the organizations Service Standards and incorporates them into the performance of responsibilities. Facilitates the continuity of care across the continuum of services, e.g. home care, other facilities, appointments with MDs etc. Provides consultation about the therapeutic regimen to the nursing staff responsible for the patients care in the hospital setting. Works collaboratively with nursing leadership to ensure timely communications with the nursing staff. Minimum Qualifications: To qualify you must have: Licensure: Current registration as Professional Nurse in New York and Registration as Nurse Practitioner in New York. Education: Master's Degree in Nursing. Certification required (as soon as feasible following graduation from NP graduate program) by ANCC or Board Certification from other specialty organization that certifies Nurse Practitioners in a clinical area of practice. Basic Life Support Certification through the American Heart Association.. Description The Medical Assistant I provides clinical office support to the supervising physician and performs patient care and administrative duties necessary to deliver quality patient care. Responsibilities 1. Fulfills patient care responsibilities as assigned which may include: checking schedules and organizing patient flow; accompanying patients to exam/procedure room, assisting patients as needed with walking, transferring, dressing, collecting and processing specimens and point of care testing, preparing for exam, etc. Assists physicians with various procedures, takes vital signs, relays instructions to patients and or families; answers calls and provides pertinent information. 2. Fulfills clerical responsibilities as assigned which may include: sending and receiving patient medical records; obtaining lab/x-ray reports, hospital notes, referral information, etc. Completing forms and requisitions as needed, managing charts to ensure information is completed and filed appropriately. These responsibilities may be carried out using the electronic medical record (EMR) system or manually as necessary. 3. Enters orders into the electronic medical record (EMR) system or prepares prescription refill requests on behalf of the physician for the review and approval by the ordering physician. 4. Inventories, orders or re-stocks medical supplies as applicable. 5. Provides set-up of examination room and instruments in accordance with proper sterilization techniques established by hospital and infection control policies. 6. Maintains logging system as required for refrigeration of medications, point of care testing, sterilization process, crash cart, point of care testing or others as needed. 7. Checks expiration dates on all medications and medical supplies disposing appropriately as per institution policy. 8. Maintains a safe, secure, and healthy work environment by following standards and procedures; complies with legal regulations. 9. Complies with established personal protective equipment requirements necessary for protection against exposure to blood and other potentially infections body fluids, chemical disinfectants, radiation and other hazardous substances. 10. Assists in the maintenance of medical charts (filing, Op Reports, test results, home care forms). 11. May schedule surgeries/procedures or work in conjunction with Surgical Coordinator verifying times with patients; preparing charts, pre-admissions and consent forms. 12. May be required to perform proficient phlebotomy duties. 13. May perform appointment scheduling and registration for patients as well as updating patient demographic and insurance information. 14. May maintain, adjust and confirm patient office schedule. 15. May check and verify and update insurance information utilizing internal or web sources. 16. Description The Medical Assistant I provides clinical office support to the supervising physician and performs patient care and administrative duties necessary to deliver quality patient care. Responsibilities 1. Overview Provides high quality nursing care and personal care to patients under the direction of the registered professional nurse consistent with VNS Health philosophy, policy, goals and objectives, and Standards of Nursing Practice. Provides nursing care to patients in any of the service areas. Description The Nurse Manager (NM) is responsible for the management of nursing practice and operations on a designated unit/service setting such as inpatient, procedural, emergency department or ambulatory care. This includes, but is not limited to, the management of: clinical nursing practice, staff recruitment, retention, labor relations, supply chain and budget, daily staffing (as supported by the Center-Led Staffing Model), employee performance evaluation, quality improvement, staff education, medical record documentation, compliance with internal and external regulatory agency requirements and patient safety solution/corrective action plans. The Nurse Manager collaborates with multiple professionals to support and coordinate the provision / management of patient care inclusive of the patient admission throughout the discharge process appropriate to the setting. This individual works closely with the Nursing Director, unit Dyad partner and staff to ensure that the Service/Unit setting maintains the highest standards of patient care and is responsive to the needs of the patient & family as well as the organization. Leads nursing practice using the tenets of the Professional Practice Model of Relationship Centered Care and the Care Delivery Model of Modified Primary Nursing for the promotion of patient experience. 2. Description RN/Case Manager MSH Case Management FT Weekdays/Weekends/Holidays The Case Manager (CM) will be responsible for all aspects of case management for an assigned group of patients to determine the appropriateness of the Level of Care, assist in the development of the plan of care; ensure that the plan is implemented in a timely basis and identify the expected length of stay (ELOS). The CM works collaboratively with physicians, social workers, clinical nurses, home care services, and other members of the interdisciplinary team as needed. Assignment will be by units/clinical areas of practice and may require responsibility on other units/services Responsibilities 1. Principle Duties and responsibilities include, but are not limited to: a. Reviews all new admissions to identify patients where utilization review, discharge planning, and/or case management will be needed using standardized criteria to achieve optimal patient outcomes and appropriate reimbursement for the organization. b. Performs continued stay reviews utilizing standardized criteria to justify continued inpatient stay. c. Collaborates with Physician and other clinicians to expedite diagnostic testing, treatment and consultations. d. Documents all clinical reviews in Case Management Documentation system. e. Supports the mission, vision, philosophy and goals of the Medical Center. 2. Case Management: The CM process will include: a. Assessment of the patients clinical, psychosocial, and functional status in collaboration with the interdisciplinary team. b. Identification and documentation of variances affecting the LOS and the discharge planning process. c. Conducts follow-up of any delays in treatment or reporting of results. d. Planning/developing specific goals with the physician, interdisciplinary team, and the patient and/or family. e. Implementation and coordination of specific activities, strategies, and interventions to move the patient through the continuum of care. f. Documentation of outcomes achieved and identified internal and external barriers. g. Identifies reasons for readmissions and collaborates with interdisciplinary team on strategies to reduce readmission rate. h. Appropriately identifies and refers cases to the physician advisor to support timely progression of patients along the continuum of care and (appropriate) discharge planning. i. Interacts with patient/family to discuss plan of care and coordination of services based on clinical needs and available resources. 3. Utilization Review: a. Maintains a working knowledge of the UR (Utilization Review) requirements of each payor within the patient population; b. Provides the clinical information requested by the managed care companies as part of the concurrent review in a timely fashion. c. Provides clinical information requested by the managed care companies in accordance with contractual agreements. d. Works collaboratively with physicians and managed care companies on concurrent denial appeals e. Communicates clinical information to the payor, as needed, coordinating direct communication between physician and payor Medical Director as required. f. In psychiatry, the CM will document UR notes in the medical record as per OMH guidelines. 4. Discharge Planning: a. Responsible for assessment, communication and monitoring of discharge planning process (The clinical nurse initiates the discharge planning process on admission). b. Obtains authorizations from managed care companies for post-discharge services. c. Assesses for clinical readiness and completes the Hospital and Community Patient Review Instrument (PRI) for patients requiring Residential Health Care Facility placement. d. Liaison with financial department for current insurance coverage. 5. Collaborates and participates in the appeals process with all members of the interdisciplinary team. 6. Collaborative Relationships: a. Develops and maintains effective working relationships with interdisciplinary team and Case Managers within the managed care organizations. 7. Description Patient Care Associate MSH GP9C Med/Surg PT (0.8) 4 shifts/wk Nights EOW As an integral part of the clinical care team, the Patient Care Associate is responsible for patient care and support activities promoting patient focused care within the Specialty Service. The Patient Care Associate supports the registered nurse in patient care activities, such as: assisting patient with activities of daily living, collecting specimens and performing procedures; maintaining a safe, clean environment; and other duties as needed. Responsibilities A. Clinical/Technical/Service • Demonstrates the ability to perform clinical/technical/service/administrative tasks • Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. • Performs and records patient’s vital signs, such as blood pressure, pulse, temperature and respirations. Coordinates with the Clinical Coordinator/Clinical Nurse Manager/Assistant Nurse Manager/Nursing Administrator to ensure supplies are appropriately utilized and inventory is sufficient for patient care. 3. Escalates material or staffing resource deficits to charge nurse, Clinical Nurse Manager/Assistant Nurse Manager or Nursing Administrator. 4. Demonstrates patient focused and cost-effective approaches to patient care in terms of equipment, staff, supplies and all other resources. 5. Documents in an accurate and thorough manner in compliance with hospital, regulatory and legal requirements and standards of care. 6. Facilitates patient throughput on a shift basis. (BSN) Associate’s Degree in Nursing (ADN) RN who is matriculated in a BSN Program with a graduation date within the year.
