Description The Medical Office Specialist offers clinical and medical office support to physicians and surgeons. Assists in patient care and administrative duties necessary to deliver quality patient care. Performs various duties as a member of the health team in the treatment and care of patient and their families and also assists with tasks to support the unit. Participates and supports the health team in the direct patient care under the supervision of an RN/LPN or MD. Responsibilities Monitors patient flow in waiting room and resolves patient flow issues related to registration and post-visit processing Provides set-up of examination room and instruments in accordance with proper techniques established by hospital and infection control policies. Takes and monitors vital signs, including fetal heart rate if necessary Performs phlebotomy, point of care testing, and other procedures as indicated by physician order, including but not limited to electrocardiogram and phototherapy Assists with procedures as necessary including but not limited to echocardiogram, sigmoidoscopy, proctoscopy, nasal scope, colposcopy, biopsy, LEEP, laser and cystoscopy Sets up oxygen and suction Performs vision screening Inventories, orders or re-stocks medical supplies; monitors expiration dates on all medications and medical supplies disposing appropriately as per institution policy Maintains logging system as required for refrigeration of medications, point of care testing, sterilization process, or others as needed Performs routine maintenance/cleaning of equipment and instruments; Notifies appropriate personnel regarding any malfunction in equipment Maintains a safe, secure, and healthy work environment by following standards and procedures; complies with legal regulations Complies with established personal protective equipment requirements necessary for protection against exposure to blood and other potentially infections body fluids, chemical disinfectants, radiation and other hazardous substances Acts rapidly and calmly during emergencies, initiates CPR and provides the necessary items called for by the team Transports patients and equipment as needed Performs any and all duties involving direct patient care as assigned by the RN/LPN within the framework of the New York State Nurse Practice Act and MSHS Policy and Procedures Documents in medical record / electronic medical record as required by hospital policy Greets and registers patients, prepares and processes appropriate forms, directs patients to appropriate areas and arranges transportation needs Screens incoming calls, takes and responds to messages, as well as responds to inquiries regarding department procedures, services and programs. Schedules appointments and completes pre-registration via telephone including insurance verification and full patient demographics collection over the telephone or in person using computerized system(s) with accountability for the integrity of the data, including all patient demographics and insurance information Fulfills clerical responsibilities as assigned using the electronic medical record (EMR) system or manually as necessary, to include sending and receiving patient medical records; obtaining lab/x-ray reports, discharge summaries, referral information, etc. Performs routine cash collections at time of service. Completes forms and requisitions as needed, managing charts to ensure information is completed and filed appropriately. Assists in the creation and maintenance of medical charts. Retrieves lab, pathology and diagnostic testing, and operative reports and data from computer systems Checks, verifies and updates insurance information utilizing internal or electronic sources. Obtains and documents managed care referrals, required pre-certifications and authorizations as necessary Supports surgical or procedure scheduling in conjunction with Surgical Coordinator or other similar role(s) verifying times with patients, preparing charts, pre-admissions and consent forms Maintains a working knowledge of New York State third party payer eligibility requirements for private and hospital billing Utilizes and understands basic medical terminology; able to complete visit encounter forms and enters ICD10-CM and CPT information into computerized systems as appropriate Assists in training, orientation and education of entry level and or/float staff Qualifications High School Diploma/GED. Medical Assistant Certification from an approved school required. Overview Provides high quality comprehensive nursing care to meet the needs of the patient/client and manages/coordinates the delivery of cost effective multidisciplinary health care services for a caseload of patients/clients consistent with VNS Health philosophy, policy, goals and objectives, and Standards of Nursing Practice. Works under moderate supervision. For Nurse-Family Partnership (NFP) only: Provides health education, social support and community linkages to meet the needs of pregnant and postpartum, first-time mothers and their infants. Also responsible for adhering to NFP national guidelines. • Assesses the physical, social and psychological needs and environmental conditions of the patient/client through professional knowledge, skills of observation, interviewing, and physical examination to develop a plan of care. • Understands, interprets, and implements reimbursement protocols; determines the billing source for patient/client care. Consults with management on complex/unique reimbursement cases. • Acts as the coordinator of care for the multidisciplinary team by providing and managing the care needs of patients/clients which includes making/confirming referrals to other disciplines/community agencies and ordering supplies/services and ensuring they are delivered as needed. • Documents content and process of all services provided in accordance with government regulations/requirements, and VNS Health policies, practices, and procedures, as well as NFP requirements as appropriate. Ensures that services follow the plan of care by ongoing assessment, planning, coordination, counseling, teaching and supervision of the delivery of care to meet the needs of the patient. • Oversees/supervises the care provided to the patient by Licensed Practical Nurses and Paraprofessionals through regular home visits in compliance with regulatory and VNS Health requirements. Assesses the patient’s condition, monitors the effect of the care plans on the patient/family health status, informs physician and other team members of changes in the client's needs and updates the plan of treatment, as needed. • Manages, plans, coordinates and executes the patient’s safe discharge from service. Ensures that physician’s orders, discharge summary and communication to team members, patient and family members adhere to VNS Health and regulatory requirements. • Communicates and consults with patient’s MD and follows-up on problematic situations affecting care such as abnormal clinical findings, medication discrepancies, or other factors that impact patient’s ability to comply with the plan of care. Manages and coordinates all aspects of assigned caseload. • Assists patients with activities of daily living, which may require positioning, moving, transferring and lifting patients of varying weights and physical conditions, with and without assistance and as appropriate, from family members and/or paraprofessionals. • For NFP Only • Establishes, implements, and revises a plan of care with input from the client and from appropriate multidisciplinary health team members (i.e., social worker, physician) based on goals mutually acceptable to the client. Contacts and interface with team members, primary physician, team physician, administrator, and other staff to ensure patient’s and family’s needs are met. • Works directly with outside vendors and care delivery partners, to ensure when patients are transferred to General Inpatient Facilities, Emergency Departments, and Paramedicine Activation, that documents and patient information to ensure continuity of care and most effective care based on patient/caregiver goals and clinical needs. • Provides clinical directions to patients/caregivers in accordance with Hospice guidelines. Obtains and receives verbal orders from team/physician and documents care using standard form. Evaluates and documents effectiveness of directions through telephone follow-up with patients/caregivers. Provides team/physician with documentation and implements any changes, as directed. • Obtains and reviews daily report from each team on significant patient care situations that may require contact/follow-up during course of shift. Documents all calls received using high degree of accuracy and detail to ensure team/physician is provided with all relevant and necessary information. Completes case conference form on all calls received and faxes to respective team. Supervises on-call Nurses: Monitors/evaluates performance, provides guidance, recommends hiring and termination, as needed. Determines/Deploys on-call Nurse for home visits, provides necessary information and follows-up regarding status of visit and to obtain necessary report. • Provides verbal report on significant events, plan of care changes and other patient/caregiver issues requiring further action and/or follow-up to each respective team. • Participates in Interdisciplinary Hospice Team meetings, management meets and serves on relevant committees for the purposes of information exchange, team collaboration, development of procedures and documentation tools, development of quality and training. • Understands/implements/complies with Hospice Care Mission, objectives, policies, and procedures to ensure Hospice Care/patient/caregiver needs are met. • Participates in special projects and performs other duties as assigned. Clinical Nurse I FT M-F 8 AM - 4:15 PM As an integral member of our Magnet Nursing team, the successful RN candidate will be focused on – patient advocacy and delivering the highest quality of care to our patients ! Responsibilities Work collaboratively with multidisciplinary health care teams to maintain the standards of professional nursing practices in a clinical setting Collect comprehensive data on patients’ health (from patients and/or family members); Analyze data to determine needs & develop individualized care plans; update care plans based on progression and pathway as well as manage care delivery Ensure that all physician orders are carried out appropriately and timely; work collaboratively with ancillary staff, including Patient Care Assistants Integrate patient care standards in all clinical activities; participate in performance improvement activities to improve patient safety, quality of care and patient outcomes Maintain sensitivity and awareness of cultural and religious diversity when addressing patients, visitors, co-workers and other customers Promote a supportive and team-oriented environment Support departmental responsibilities consistent with the long-range strategic plan, mission, vision and values of the hospital Minimum Requirements BSN from an accredited school of nursing required 1+ years’ of acute care experience NJ RN license and registration required Excellent phlebotomy skills BLS certification required Essential Qualifications HCAHPS/EPIC knowledge preferred Your scope of duties will include medical-surgical clinical competence, incorporating physical and emotional assessments, age specific competency, critical thinking skills, and educating patients and their family members regarding their plan of care and clinical pathway.
