Within the Emergency Department to proactively identify potential patient admissions and patients with needs for discharge. Works with Bed Center staff to identify inappropriate patient status issues (inpatient versus observation). Resolves issues of incorrect status through discussions with ED and Admitting Physicians by obtaining additional information to support admission order or determine alternative level of care. Coordinates discharges home from the ED and facilitate transfers to external placement facilities.
