Responsible for leading and managing quality improvement initiatives impacting core clinical processes and critical patient care functions across all Outpatient Surgical Sites. Responsible for developing, coordinating, leading, and analyzing clinical initiatives to drive optimal patient outcomes and improve the patient experience. Acts as a role model and champion for a culture of safety. Works collaboratively with executive leadership, administrators, directors, managers, physicians and other interprofessional colleagues to identify and enhance strategic clinical issues which produce quantitative and qualitative results for both internal and external Medical Center customers. Oversees and monitors the development and implementation of quality initiatives across Outpatient Surgical Sites. Provides effective project management/leadership to ensure completion of Medical Center Quality and Performance Improvement to meet goals and objectives and regulatory standards set by TJC and AAAHC. Clinical Expertise: Demonstrates clinical expertise in specific patient population(s) related to projects managed. Technical Execution: Demonstrates technical execution in collection, verification, and analysis of data. Leadership & Training/Development: Provides leadership and training/development to project team members. Communication & Interpersonal Skills: Communicates effectively with all levels of internal and external personnel. Willing to travel between locations, as part of the regular working day. In addition to the above job responsibilities, other duties may be assigned. MINIMUM REQUIREMENTS: Education: BSN required; MSN preferred. Licensure: Current Virginia nursing licensure required. Experience: At least 3-5 years of perioperative nursing experience with progressive quality improvement or leadership responsibilities. Proficient communicative, auditory, and visual skills; Attention to detail and ability to write legibly. The pay range for this role is $67,859.00 - $135,718.00 annually. A dynamic professional clinician focused on population management with demonstrated capability to provide relationship-based care within a collaborative multidisciplinary team. This registered nurse utilizes advanced critical thinking to develop, implement, and evaluate an individualized plan of care that educates, empowers and optimizes patient outcomes throughout all transitions within the continuum of care. In addition, this position requires clinician provide afterhours/weekend coverage to serve as primary point of contact for pre and post-transplant patients and internal and referring providers. Business and after hours care coordinator responsibilities include telephone assessment using strong clinical knowledge base, complex care coordination and treatment based on comprehensive assessment and strong interpersonal communication skills, calling in patients for transplant, and ensuring all multidisciplinary members of the transplant team are kept informed of clinical decisions and patient response to interventions in a timely manner. Transplant coordinators must be knowledgeable of the principles of ethical practice and have resources available to evaluate the merits, risks, and social concerns of transplantation. Participates in Transplant Center Quality Assurance & Performance Improvement (QAPI) program meetings, projects and events. Experience: 2 years of relevant experience. Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Health Care Provider BLS certification required. Relevant specialty board certification strongly preferred. PHYSICAL DEMANDS Job requires standing for prolonged periods, frequently traveling, bending/stooping.
