Job Requirements The Care Manager collaborates with members of the healthcare team, community resources, and with the patient/family to promote optimal outcomes for the patient across the continuum of care. The Care Manager provides Care Management activities to ensure an effective, appropriate, individualized and safe plan of care. Work Experience Education: Graduation from an accredited School of Nursing or college with current nursing licensure in the State of Maryland. BSN or Master prepared preferred.
