SUMMARY Provides medical services to the patients and families, assists physicians and mid-level practitioners (providers) with patient care. To perform all clinical functions in a physician’s office. To include but not limited to: Assisting the physician with treatments, obtain specimens from patients and process properly, maintaining medical information and filing/scanning, maintain sample supplies, ordering and maintaining clinical supplies as needed, patient education, obtaining histories and vitals from patients, triage patient calls, prescription refills, call backs for normal or abnormal results as per physician direction, prior authorizations as needed, telephone answering and appointment scheduling as needed. MINIMUM EDUCATION, KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED Must be a graduate of a LPN program. Must possess great attention to detail. Must be able to accurately type and be efficient with data entry equipment. Must have the ability to apply appropriate rules of grammar, spelling and punctuation. Knowledge of computers is a necessity. Must possess the ability to prioritize activities, organize work and problem solve. Communications skills both verbal and written must be above average to excellent. Must be able to communicate with public sector by telephone use and in person. Must be able to deal with anxious and angry people. Must be able to communication with Administrative Staff, Managers, Attorneys, Physicians and all hospital staff with due respect and tact. SUMMARY Coordinate total nursing care for patients using nursing standards and the nursing process; adhering to Oncology Nursing Standards. Participate in patient and family teaching; provide leadership working cooperatively with other health care providers. JOB DUTIES Reports pertinent observations, changes, and reactions regarding patients to the appropriate person (physician, manager, charge nurse, supervisor, etc.) Records these observations, changes and reactions accurately, concisely and legibly. Plans, implements and documents health teaching with patient and/or significant other. Assesses the patient’s condition and nursing needs. Based on those conditions and needs, develops and implements individual plan of care with revisions as patient’s condition and needs change. Assess pain, fatigue, nausea and vomiting specific to diagnosis and therapy. Maintains the standards of nursing care within the guidelines of the nursing department. Demonstrates knowledge of and implements policies and procedures of the hospital and nursing departments. Directs, supervises and evaluates nursing care provided to the patients and administers direct care, by using professional nursing skills and judgments as required. Includes pain assessment, interventions, and re-assessment. Assists physicians with patient procedures, and exams. Transcribes physician orders accurately and legibly. Administers medications, including IV’s according to hospital policy and procedure. Administers chemotherapy and biologic therapies according to hospital policy and procedure. Demonstrates cost-conscious use of supplies; maintains equipment in a clean and safe manner. Participates in the orientation of new staff and in the clinical experience of students. Assumes personal responsibility for professional development and continuing education. Oncology Nursing Society (ONS) Chemotherapy provider course with recertification every two years; ONS certification preferred with recertification every four years. Special computer and use of electronic medical records skills utilized. Recognizes and reports findings outside the normal range for this age group. Observes lab values pertinent to specific diagnosis and therapy. Reports abnormal to physician. Ability to communicate and serves as a liaison between patient’s family and services needed. Knowledgeable of importance of patient safety assessment. Performs other duties as assigned. Color Vision: Frequently - Ability to identify and distinguish colors. MINIMUM EDUCATION, KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED Professional knowledge of nursing theory and practice at a level normally acquired through completion of an accredited School of Nursing. Two to three years oncology experience, competent intravenous skills. Completion of Comprehensive Cancer and chemotherapy course for new oncology nurses. ONS chemotherapy provider card required within 6 months of employment. OCN certification preferred. Current State Nursing License. Communication skills necessary to effectively: Provide instruction to the patient and/or significant other Provide explanations to the patient and/or significant other Explain nursing intervention Communicate with other members of the health team. BLS required. WORKING CONDITIONS Normal patient care environment with little exposure to excessive noise, dust, and temperature changes. A large part of work time is standing or walking. Subject to certain distasteful patient care activities and exposure to communicable diseases. May be scheduled overtime, weekend and/or holiday hours. May be exposed to potential hazards such as chemical wastes, body wastes, fluids and other possible infectious material. Adherence to Oncology Nursing Guidelines and use of specially designed chemotherapy personal protective equipment is required per hospital policy and procedure. Essential Job Functions • Under the direction of a Licensed Professional (e.g., Physician, Physician Assistant, Nurse Practitioner, or Registered Nurse), participates in the nursing process to include data collection, planning, implementation, and evaluation of the patient’s plan of care accurately and in a timely manner while collaborating with other nurses and members of the healthcare team. • Gives direct nursing care and assists with care of the patient at the direction of a Registered Nurse, Licensed Physician, Physician Assistant, or Advanced Practice Nurse. • Contributes to the plan of care with input from patient/family and other members of the healthcare team. • Performs patient data collection and observation considering patient's age group, severity of condition/disease (including physical, psychosocial, cultural, environmental, self-care, and education needs), and coordination of resources and referrals. • Collect and document objective data, subjective data, and patient care including focused observations such as pain, pain relief, results of treatments, updates to plan of care, implementation, and evaluation of care following unit specific and hospital charting guidelines. • Implements patient care in a safe, knowledgeable, skillful, consistent, and continuous manner. • Administers ordered medications and treatments, within scope of care, prescribed by a Licensed Physician, Physician Assistant, or Nurse Practitioner using appropriate medical equipment as needed. • Performs phlebotomy services • Contributes to the evaluation of the patient’s response to nursing interventions, involving the patient, family, significant others, or other members of the health care team appropriately; documents and communicates patient’s responses to appropriate members of the healthcare team. • Recognizes and reports significant changes in the patient's condition and contributes to plan of care revision based on ongoing evaluation. • Demonstrates sensitivity to the needs of a diverse patient population. • Adheres to the safety policies and procedures of the organization. The Psychiatric Nurse Practitioner is responsible for providing comprehensive psychiatric services to incarcerated individuals within a correctional facility and with a collaborating physician. This includes assessing, diagnosing, and treating psychiatric disorders and collaborating with a multidisciplinary team to ensure the delivery of quality healthcare services within a medical home model. The successful applicant should be able to perform all of the following functions at a pace and level of performance consistent with the actual job performance requirements: Psychiatric Assessment and Diagnosis: Conduct thorough psychiatric evaluations and assessments of incarcerated individuals to determine the presence and severity of mental health disorders. Diagnose and classify mental illnesses according to established diagnostic criteria (e.g., DSM-5). Assess and evaluate suicide risk, self-harm, and dangerous behaviors among incarcerated individuals. Collaborate with other healthcare professionals and correctional staff to gather relevant information for accurate diagnoses. Treatment Planning and Implementation: Develop and implement individualized treatment plans for incarcerated individuals with psychiatric disorders. Prescribe appropriate psychiatric medications and monitor their effectiveness and side effects. Provide brief psychotherapy and counseling services to incarcerated individuals, using evidence-based therapeutic modalities. Collaborate with the interdisciplinary team to coordinate care and ensure a holistic approach to treatment. Modify treatment plans as needed based on patients' progress and response to interventions. Crisis Intervention and Emergency Services: Respond to psychiatric emergencies within the correctional facility promptly and efficiently. Conduct crisis assessments, provide stabilization, and implement appropriate intervention strategies. Collaborate with correctional staff and healthcare team members to implement emergency response protocols. Determine the need for a higher level of psychiatric treatment when necessary. Collaboration and Consultation: Collaborate with other healthcare professionals, including primary care physicians, psychologists, social workers, and nurses, to provide integrated care to incarcerated individuals. Consult with the interdisciplinary team to develop comprehensive treatment plans for incarcerated individuals with complex psychiatric needs. Provide expertise and guidance to correctional staff regarding mental health issues, crisis management, and suicide prevention. Participate in case conferences, treatment team meetings, and quality improvement initiatives. Documentation and Compliance: Maintain accurate and up-to-date psychiatric records for each incarcerated individual, including assessments, diagnoses, treatment plans, and progress notes. Ensure compliance with relevant laws, regulations, and ethical guidelines pertaining to correctional healthcare and patient confidentiality. Adhere to the correctional facility's policies and procedures related to documentation and record-keeping. Education and Training: Stay updated with the latest advancements in psychiatric medicine, treatment modalities, and relevant research. Engage in continuous professional development and relevant educational activities. Provide training and education to correctional staff on mental health topics, crisis management, and de-escalation techniques Qualifications: Board certification as a Psychiatric Mental Health Nurse Practitioner Current and unrestricted Nurse practitioner license in the state/jurisdiction where the correctional facility is located. Experience in providing psychiatric care or relevant experience in correctional or underserved populations is preferred. Strong assessment and diagnostic skills, with the ability to manage a wide range of acute and chronic medical conditions. Proficiency in prescribing medications and therapies under physician supervision A valid driver's license is required. Minimum age requirement: Must be at least 18 years of age.
