Position Summary: Vitae Health is seeking a dedicated Nurse Practitioner to provide high-quality, patient-centered medical care to residents in a skilled nursing facility. The NP will assess, diagnose, and manage acute and chronic conditions while collaborating with an interdisciplinary care team to improve patient outcomes and quality of life. In this role you will partner with the rest of the clinical team to provide patients the highest quality of care. Under the direction of a faculty sponsor and in collaboration with the program support team, coordinates the care of patients through the disease process. Acts as a clinical liaison for patients, across medical services and clinical departments. Assesses clinical, emotional, spiritual, psychosocial, financial, and other patient needs. Evaluates and manages patient symptoms before during and after treatment. Essential Job Functions: Provides care coordination & patient education to patients in MSK service line. Supports discharge planning for the MSK service line. Supports transitioning of trauma/OON patients to providers within network.
