Provides skilled nursing care to hospice clients in their homes or community health settings ensuring the delivery of outcome based, high quality, cost-effective care. · Reviews referrals, and initiates contact with client and family. · Conducts a complete nursing assessment based on referral data, client/family interview, and clinical observation of patient health status. Demonstrates clinical expertise in symptom management and end of life issues. · Develops a plan of care in collaboration with client and all disciplines based on assessment data which documents need for services, appropriate discharge planning, short- and long-term goals, and the utilizations of appropriate resources. Facilitates peaceful death and assures appropriate bereavement follow up through timely documentation of bereavement assessment. Modifies plan of care as conditions change. · Demonstrates understanding of payer information in order to maximize care planning. Utilizes Hospice Levels of care appropriately. · Follows IDT process of approving non-formulary medications, treatments, and DME. · Initiates and maintains a client record utilizing the Agency’s standard for documentation. · Makes follow-up visits to provide skilled care to assess disease progress and evaluates response to treatment so as to promote peaceful death; provides support to client/family utilizing teaching principles to promote and restore health and prevent disease and disability, as possible; coordinates care with providers. Utilizes interdisciplinary team members to support patient/family needs. · Teaches patients and reinforces client/caregiver teaching and education. Prepares/teaches/supports family regarding signs and symptoms of death, as appropriate. · Refers, collaborates, and coordinates all client care services. Consults with physician regarding client needs and plan of care. · Directs LPNs in carrying out the established plan of care. Directs Home Care Aides and supervises plan of care. This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. PRINCIPAL DUTIES AND RESPONSIBILITIES: All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care policy, procedures, standards of nursing practice, state and federal regulations. · Performs all essential functions under the direction of the Supervisor and with guidance from the Clinical Educator, Preceptor or in collaboration with other Registered Nurses. · Performs ongoing, systematic collection and analysis of patient data pre – during – post hemodialysis treatment for assigned patients and documents in the patient medical record, makes adjustments or modifications to treatment plan as indicated and notifies Team Leader, Charge Nurse, Supervisor or Physician as needed. · Assesses, collaborates, and documents patient/family’s basic learning needs to provide initial and ongoing education to patients and family. · Directs and provides, in collaboration with direct and ancillary patient care staff, all aspects of the daily provision of safe and effective delivery of hemodialysis therapy to assigned patients. · Administers medications as prescribed or in accordance with approved algorithm(s), and documents appropriate medical justification and effectiveness. · Initiates or assists with emergency response measures. · Serves as a resource for health care team, participates in staff training and orientation of new staff as assigned. · Ensures correct laboratory collection, processing and shipping procedures are performed and reschedules missed or insufficient laboratory collections. · Identifies expected outcomes, documents and updates the nursing assessment and plan of care for assigned patients through collaboration with the Interdisciplinary Team. · Ensures patient awareness related to transplant and treatment modality options. · Required to complete CAP requirements to advance. · Performs all other duties as assigned by Supervisor. The Greater Manchester Region serves a region of 300,000 people in 19 towns. POSITION SUMMARY: Utilizing the nursing process, provides high quality direct and indirect patient –centered care. Coordinates patient care by working collaboratively with the interdisciplinary team, patients and families. Communicates, as appropriate, the patient’s clinical condition, responding quickly and accurately to changes in condition or response to treatment. Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age appropriate care to the patient population served. Promotes the profession of nursing and professional practice through collegial support and interactions. Reports to the Clinical Nurse Manager. COMPETENCIES: Must be able to communicate effectively in English, both verbally and in writing and possess basic computer skills. ESSENTIAL DUTIES and RESPONSIBILITIES: Job-Specific Competency 1. Performs population-specific comprehensive assessments and re-assessments of the patient’s condition and care needs including the psychosocial, physical, educational and cultural aspects. 2. Utilizing assessment data, collaborates with surgical team to define, implement and evaluate an individualized plan of care for each surgical patient. 3. Follows the five (5) medication rights and reduces the potential for medication errors. Demonstrates knowledge of medications and their correct administration based on age of the patient and their clinical condition. Follows Surgical Services policy for medication safety. 4. Performs all aspects of patient care in an environment that optimizes patient safety and promotes positive patient outcomes; follows ECHN policy for Patient Identification, Surgical & Invasive Procedures Site Identification, and Time-Out. 5. Documents the nursing process and responses to care in a timely manner utilizing the appropriate tools. 6. Demonstrates an ability to be flexible, organized and function under stressful situations. Utilizes critical thinking skills and sound judgment in priority setting and delegation. 7. Demonstrates autonomous practice, consistent with evidence-based standards. Pursues personal and professional growth and development. Serves as a professional role model and mentor. 8. Demonstrates knowledge of the principles of growth and development over the lifespan and the skills necessary to provide age-appropriate care to patients served. Able to interpret data about the patient status in order to identify each patient’s age-specific needs and provide care needed by the patient group. 9. Provides an accurate and pertinent report of the patient’s condition for safe transfer of care. Prepares room in a timely manner with all anticipated instruments, sutures, supplies, and equipment for each surgical procedure. Demonstrates knowledge of and assures maintenance of aseptic technique throughout the surgical procedure; identifies and communicates, to surgical team, any breaks in technique and assists with corrective actions. Effectively communicates with the surgeon in accounting for all sponges, needles, and instruments; reports discrepancies immediately to surgeon and Team Leader; appropriately documents all counts. Accurately records, handles, labels and processes specimens. Demonstrates ability to effectively function in the scrub role. Qualifications EDUCATION/CERTIFICATION: Must be a graduate of an accredited school of Nursing and have and maintain current a Registered Nurse license in the State of Connecticut. Must have certification in Cardio Pulmonary Resuscitation (CPR) upon hire. EXPERIENCE: Previous surgical or labor and delivery, specifically assisting with C-sections, experience required. Provides general or specialized nursing care to patients in ambulatory setting or clinical practice. Provides nursing services to patients and families in accordance with the standards of practice of a professional nurse, Connecticut Children's policies and procedures and State and federal regulations. Reports to Nurse Manager. Prioritizes, plans, provides direct family-centered care to patients within area of service throughout the continuum of care utilizing time and resources effectively and efficiently. Facilitates the flow of patients within the clinical practice. Partners with child and family in evaluating outcomes. Monitors, trends and evaluates child/family responses to treatment throughout scope of service. Provides appropriate education to patient/family regarding all aspects of care, including telephone advice, based upon established protocols. Serves as advocate for parents and families. Clearly documents all pertinent aspects of care, including: a plan of care; a regular summary of patient status and family adjustment; unexpected events of changes in patient condition; medications; and monitoring of vital signs, etc. Complies with CCMC policies regarding appropriate documentation. Delegates aspects of care to peers and ancillary personnel to meet needs. Serves as a resource to staff within area of expertise. Networks with healthcare team by documenting and communicating comprehensive data including complex child/family needs to provide continuity. Provides psychosocial support for patient, family and peers to promote healthier patient/family/team outcomes. Follows "Comfort Central" as the guiding principle in the assessment and treatment of pain, as outlined in the management of pain policy. Demonstrates knowledge of the age-related differences and needs of patients in appropriate, specific populations from neonate through adolescence and applies them to practice. Demonstrates cultural sensitivity in all interactions with patients/families. Demonstrates support for the mission, values and goals of the organization through behaviors that are consistent with the CCMC STANDARDS. Education and/or Experience Required: A minimum of ASN/ADN required upon hire; BSN preferred. If ASN/ADN, must acquire BSN within 3 years of hire License and/or Certification Required: Current State of Connecticut Registered Nurse licensure or Nursing Compact License required and maintained thereafter. BLS/CPR for Providers from American Heart Association, or American Red Cross, preferred upon hire and required within 1 month of hire. Must remain current at all times. Specialty licensure/certification may be required, as specified by department. Knowledge, Skills and Abilities: Able to demonstrate all of the CCMC Core Organizational Competencies or has potential to do so as well as any clinical competencies within specialty area.
