Description The Clinical Program Manager is responsible for assuring compliance with mandated clinical orientation, competency assessment and interventions in the assigned clinical program of designated nursing staff within the Medical Center. This individual interfaces directly with MSMC Departments, Administrators, Nursing, Medical and Quality Leadership, and Clinical Staff, managing the educational and operational impact of all clinical care delivery, staff performance, and outcome measures to support clinical practice. Responsibilities 1. Oversees clinical operations within the program, using data and fact-based problem solving techniques to improve processes and outcomes. Ensures that all clinical initiatives run according to institutional and departmental policies and in accordance with any government or regulatory requirements as applicable. 2. Recruits, orients, trains, coaches, counsels, mentors, disciplines, and evaluates staff in accordance with all internal policies and procedures. Communicates values, strategies, and objectives of department or division on a regular basis. Assigns accountabilities, delegates tasks and responsibilities, and plans, monitors and appraises job performance. 3. Develops, maintains and reconciles departmental budget with financial responsibility and oversight for department or division. 4. Plans, develops, and/or maintains departmental standard operating procedures. Regularly reviews policies and procedures and makes changes, or recommends changes to superiors as necessary. 5. In conjunction with Nursing, Medical and Quality Leadership, coordinates educational and performance improvement activities within a designated clinical specialty. 6. Develops and designs of action plans to support clinical performance and competency and provides support and educational resources to departments and services relative to nursing care delivery within the context quality assessment and improvement processes. 7. Establishes and implements short and long term goals, objectives, policies and operating procedures for high level of performance in nursing care delivery. 8. Coordinates and leads in the execution of day to day educational program activities, as appropriate to program objectives and areas of clinical expertise. 9. Oversees the collection, compilation, and analysis of clinical program activity data to evaluate clinical performance and competence. 10. Provides technical assistance, resource services, consultation and education to clinical specialty service relative to orientation, ongoing assessment of competency and performance improvement activities. 11. Serves as a clinical role model and resource to the nursing staff. 12. Prepares reports and analysis associated with orientation of new hires, ongoing competency and evaluation of staff performance and identification of related outcome measures. 13. Provides timely status reports of results of assignments including identification of patterns and trends that require actions. 14. Collaborates with other members of the health care team in the development, implementation, and ongoing review of policies, procedures, and standards of care for designated patient populations. 15. Demonstrates knowledge of hospital, departmental and care center standards, policies, procedures and guidelines as well as regulatory agency standards. Assures readiness for Joint Commission and other agency surveys and presents related programs during Joint Commission and other agency visits. 16. Participates in daily activities that reflect budgetary constraints and incorporates financial principles into program development and allocation of resources. 17. Maintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; benchmarking state-of-the-art practices; participating in professional societies. 18. Participates in the performance appraisal process of designated Nursing staff. Qualifications Master's Degree in Nursing is required. NP Certification in specialty area required. 5 years directly related experience (some supervisory preferred) New York State License with current registration as a registered professional nurse. Certification in specialty area preferred. Description Provides clinical office support to physicians and surgeons and performs patient care and administrative duties necessary to deliver quality patient care. Responsibilities 1. Description Job Title: Patient Care Associate-Rehab-Full Time Day 7am-3:15pm-MSM As an integral part of the clinical care team, the Patient Care Associate is responsible for patient care and support activities promoting patient focused care within the Specialty Service. The Patient Care Associate supports the registered nurse in patient care activities, such as: assisting patient with activities of daily living, collecting specimens and performing procedures; maintaining a safe, clean environment; and other duties as needed. Responsibilities A. Clinical/Technical/Service • Demonstrates the ability to perform clinical/technical/service/administrative tasks • Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area. • Performs and records patient’s vital signs, such as blood pressure, pulse, temperature and respirations. Documents intake and output and other data as assigned • Obtains patient’s rating on pain scales as a fifth vital sign in all areas except Pediatrics • Performs clinical procedures, such as specimen collection including blood glucose, venipuncture, EKG, application of compression devices, post-mortem care, and maintenance of skin integrity. • Sets up for selected procedures and assists RN’s accordingly. • Assists patient with activities of daily living, such as feeding, bathing and/or toileting. Orients Patients to clinical environment. Assists patient with ambulation and all levels of mobilization. • Assists with dietary functions, including delivering and collecting patient trays at bedside, passing menus to patients and assisting them with menu completion, mixing nutritional supplements, reporting variation in eating patterns to RN/Dietitian, and maintaining accurate and timely systems for transmission of patient dietary orders and changes • Maintains a safe, clean and functional environment, including ensuring for the comfortable Movement of the patient. Assists the patient in room maintenance, such as bed-stripping and trash removed, when necessary • Performs on-to-one observation or close observation status of patients, e.g. safety, elopement risk, or suicide prevention. • Maintains “isolation status” for patients under direction of an RN and according to set procedures • Performs scheduled hourly care checks/ rounding on patients with emphasis on caring for toileting needs, comfort, and positioning and pain awareness. Overview Oversees, supervises, coordinates, facilitates, and supports the Hospice After Hours care delivery activities, including field staff supervision. Manages continuity of care to all Hospice patients and their families after normal operating hours. Responds to incoming calls from patients/families/caregivers, assesses and priorities patient needs based on policies and procedures and staff capacity, ensuring all emergent and urgent request are met timely. Addresses needs within context of patient's Interdisciplinary Plan of Care and Hospice Care mission, objectives, policies, and procedures. Documents and reports to team patient's needs, significant events, plan of care and other issues. Evaluates effectiveness of clinical directions. Works under general supervision. Ensures workload assigned is appropriate to staff’s ability and capacity in order to provide quality and comprehensive services; provides clinical oversight of nurses through coordination and coordination, and feedback from staff and patients/caregivers. Maintains current knowledge of clinical standard practices and the use of VNS HEALTH technology, to ensure care delivered is of the high quality, while maintain cost efficiency. Assesses the physical, emotional, and medical needs of hospice patients and their caregivers through after-hours telephone consult. Provides support and direction to families in accordance with the patient’s Interdisciplinary Plan of Care and Hospice Care mission, objectives, policies, and procedures. Takes referral information and triages for immediate response, dispatches the on-call nurse to do the evaluation/ admission. Contacts Physician for verification of prognosis and appropriateness for Hospice/Palliative care. Contacts and interface with team members, primary physician, team physician, administrator, and other staff to ensure patient’s and family’s needs are met. Works directly with outside vendors and care delivery partners, to ensure when patients are transferred to General Inpatient Facilities, Emergency Departments, and Paramedicine Activation, that documents and patient information to ensure continuity of care and most effective care based on patient/caregiver goals and clinical needs. Provides clinical directions to patients/caregivers in accordance with Hospice guidelines. Obtains and receives verbal orders from team/physician and documents care using standard form. Evaluates and documents effectiveness of directions through telephone follow-up with patients/caregivers. Provides team/physician with documentation and implements any changes, as directed. Obtains and reviews daily report from each team on significant patient care situations that may require contact/follow-up during course of shift. Documents all calls received using high degree of accuracy and detail to ensure team/physician is provided with all relevant and necessary information. Completes case conference form on all calls received and faxes to respective team. Supervises on-call Nurses: Monitors/evaluates performance, provides guidance, recommends hiring and termination, as needed. Determines/Deploys on-call Nurse for home visits, provides necessary information and follows-up regarding status of visit and to obtain necessary report. Provides verbal report on significant events, plan of care changes and other patient/caregiver issues requiring further action and/or follow-up to each respective team. Participates in Interdisciplinary Hospice Team meetings, management meets and serves on relevant committees for the purposes of information exchange, team collaboration, development of procedures and documentation tools, development of quality and training. Understands/implements/complies with Hospice Care Mission, objectives, policies, and procedures to ensure Hospice Care/patient/caregiver needs are met. Participates in special projects and performs other duties as assigned. Qualifications Licenses and Certifications: Hospice and Palliative Nurse (CHPN), Hospice and Palliative Credentialing Center preferred License and current registration to practice as a Registered Professional Nurse in the State of New York required Education: Associate's Degree in Nursing required Bachelor's Degree in Nursing preferred Work Experience: Minimum of two years of clinical nursing experience in Acute Care (Med Surg, ICU), Clinical Crisis Management, Hospice or Palliative Care required Clinical expertise and critical thinking skills required Effective oral, written, and interpersonal communication skills required Demonstrated expertise in clinical care management required Knowledge of personal computer operations including MS Office applications required Experience in health care management preferred Pay Range USD $93,400.00 - USD $116,800.00 /Yr.
