POSITION OVERVIEW The Resident Care Associate (RCA) is responsible for providing general non-certified routine services in accordance with the established procedures and may be directed by your supervisors. This role specifically supports our Assisted Living, AL Memory Care and Independent Living residents. Key Responsibilities Assists with housekeeping and laundry services including but not limited to, trash removal and making and changing unoccupied beds. Provides resident assistance with activities of daily living including but not limited to hygiene care, incontinence assistance, bathing & grooming. Assists with all aspects of meal service including but not limited to, transportation of residents to and from dining room, tray delivery, and ensuring great dining experience. Answer residents call lights promptly. Ensures that residents who are unable to call for help are checked frequently. Assists clinical staff to make residents comfortable (i.e., check sitting position, hydration passing, etc.) Assists with personal care responsibilities according to state regulatory guidelines. Location BBC Bronson Battle Creek Title Registered Nurse ( RN ) - Bronson Battle Creek Birthplace - OB / Labor Delivery / Mother Baby The Bronson Birthplace in Battle Creek has 15 beds – 11 for labor/delivery/recovery/postpartum (LDRP) care, 4 for antepartum/post-op care, 2 operating rooms and 3 triage bays. Additionally, the unit includes a newborn nursery. Here, pregnant, laboring and postpartum patients, as well as healthy newborns, receive comprehensive care. RNs on this unit are able to undergo cross-training in labor and delivery, including operating room procedures, postpartum care and nursery care. Under general supervision and in accordance with the policies, procedures and guidelines established within the organization, the unit, and the Michigan Public Health Code, the registered nurse manages the care delivery for patients in the organization throughout the length of stay to achieve quality outcomes and timely discharge by the following: utilizing the nursing process of assessment, planning, diagnosing, implementation and evaluation; coordinating the care provided by other professionals; delegating selected acts, tasks or functions that fall within the scope of professional nursing practice and the abilities of patient care assistants and universal service assistants; providing selected aspects of physical care within the scope of professional nursing practice as appropriate given the patient's level of acuity and complexity of care required; and documenting and communicating during each tour of duty the patient's response to care provided, consumption of resources and anticipated needs. Position Summary: The Certified Nursing Assistant provides quality nursing care to residents. They coordinate work within the department, as well as with other departments, report pertinent information to the immediate supervisor, and assists immediate supervisor with tasks to support department operations. Principal Duties and Responsibilities: Provides quality nursing care to residents in an environment that promotes their rights, dignity, freedom of choice and individuality. Provides individualized attention, which encourages each resident’s ability to maintain or attain the highest practical physical, mental and psychosocial well-being. Is knowledgeable of the individualized care plan for residents and provides support to the residents according to their care plan. Contributes to the care planning process by providing the charge nurse or other care planning staff with specific information and observations of the residents’ needs and preferences. Maintains the comfort, privacy, and dignity of residents in the delivery of services to them. Interacts with residents in a manner that displays warmth and promotes a caring environment. Tidy rooms and change bed linen as necessary. Fully understands all aspects of residents’ rights, including the right to be free of restraints and free of abuse. Is responsible for promptly reporting to the charge nurse or administrator incidents or evidence of resident abuse or violation of residents’ rights. Assists in maintaining a safe, neat and clean environment; reports environmental deficiencies to the Environment Plant Manager such as lighting or equipment problems. Protects the personal belongings of each resident, including eyeglasses, dentures, hearing aids, furnishings, jewelry, clothing and memorabilia. Promptly reports missing items according to established policy and participates in efforts to locate missing items. Observes residents for changes in condition or behavior and promptly reports these changes to the Charge Nurse. Provides care that maintains each resident’s skin integrity to prevent pressure ulcers, skin tears and other damage by changing incontinent residents, turning, repositioning immobile residents and by applying moisturizers to fragile skin and other areas. Answers residents’ call lights promptly and courteously. Performs various tasks assigned by the charge nurse, including checking vital signs, weighing residents, applying creams/ointments and collecting specimens. Assists with orienting residents and their families to the nursing home on admission and to the unit when transfers occur. Lifts, moves, and transports residents, using proper body mechanics or lifting devices for accident prevention. Also use wheelchairs, slide boards, walkers, and canes as required. Communicates and interacts effectively and tactfully with the residents, visitors, families, peers and supervisors. Assists and escorts residents for appointments such as at the beauty shop or attending activities or church services. Participates in activities and functions as directed. Completes documentation of care provided or other information in PCC in accordance with department policies. Performs all job responsibilities in accordance with prescribed safety and infection control procedures, including thorough hand washing, use of disposable gloves where indicated and proper disposal of soiled materials Attend staff meetings and in-services as requested. Answer incoming calls when necessary and direct calls appropriately. Perform other tasks as required. Required/Desired Qualifications: Education, Training, and Experience: Must possess a current Michigan Certified Nurse Aide Certification or have completed a Michigan approved CNA training program and are eligible to test. Must be certified within 4 months of hire. One year of experience in the long-term care, hospice, or health care desired. Position Summary: DAY SHIFT Weekend Warrior $8.00 premium pay The Registered Nurse is responsible for performing a variety of duties to provide quality nursing care to residents and to coordinate total nursing care for residents; for participating with the Director of Nursing and the Unit Managers in establishing specific goals; for ensuring compliance with all operating policies and procedures and Michigan Department of Health & Human Services regulations and Federal Health Care Administration regulations; for communicating with and supervising appropriate personnel; for providing daily reports and other periodic reports; and for performing all the duties of a Certified Nurse Aide when required. Principal Duties and Responsibilities: Assess resident conditions to implement intervention and develop Care Plans. Accurately record resident observations in PCC and follow through on Care Plans. Assure facility medical records policies and procedures are adhered to. Make regular rounds of residents independently and with a physician; observe and evaluate resident symptoms, progress, and reactions to treatments and medications and implements interventions as necessary. Provide preventative, supportive, maintenance, and rehabilitative care directed to the physiologic and psychosocial needs and well-being of residents. Provide care to residents by performing a variety of treatments, including changing dressing, cleansing and/or irrigating wounds and incisions, performing Foley Catheter irrigations, IV therapy, and administering tube feedings, chest tubes and peritoneal dialysis. Request and assure supplies and medical equipment are available to perform care and treatments as required by the resident. Schedule appointments, physicians, therapy, and consults. Document in appointment book. Monitor and assist CNAs with personal resident care duties such as bathing, feeding and assisting with ambulation, and other routine accommodative services for resident comfort and well-being. Receive reports from outgoing nurse and make rounds to see each resident daily and as needed. Administer medications to residents according to the Public Health Code, Nursing Department Policies, and standards and procedures and as prescribed by the physician; notify appropriate clinical and/or nursing personnel of medication contraindications. Advise residents and/or responsible persons regarding changes in medications, treatment plans, and resident condition. Manage and document emergency care of residents, coordinating communications to physicians, responsible persons, DON and Administrator, hospital personnel, emergency transport, and other appropriate personnel. Perform comprehensive and focused nursing assessments in conjunction with changes in condition and scheduled programs. Admit, transfer, and discharge residents. Requests and assures supplies and equipment are available to perform care and treatment as required by the patient prior to or at the time of admission. Assure completion of weekly, periodic and discharge summaries. Perform quality improvement activities as assigned by the Director of Nursing. Monitor rate of accident/incidents occurrences and infection rate on the unit and develop suggestions for corrective action. Weekly skin assessment to monitor occurrence of pressure injuries or other skin conditions, providing the Director of Nursing written data on the number of occurrences. Report resident and staff complaint and all unusual occurrences to the Director of Nursing or Administrator and any allegation of Abuse/misappropriation or neglect to the administrator immediately. Participate in periodic regulatory surveys by the State of Michigan and participates in the preparation of the Plan of Correction response to an inspection survey. Complete and report resident injury to Unit Manager. Fully understands all aspects of residents’ rights, including the right to be free of restraints and free of abuse. Is responsible for promptly reporting to the charge nurse or administrator incidents or evidence of resident abuse or violation of residents’ rights. Know and support facility philosophy, standards, policies and procedures. Communicate with department heads on an as-needed basis to problem solve and plan for more effective patient care. Inform Director of Nursing when staffing requirements are not met. Present a professional appearance and serve as unit role model. Maintain own professional requirements for continuing education and staff development by participating in work related seminars, etc. Promotes compliance with accident prevention procedures, safety rules, and safe work practices to prevent employee injury and illness and control worker’s compensation costs. Ensure equipment is used in a safe manner and report any maintenance concerns to the Environment Plant Manager. Adheres to the Nurse Practice Act. Attend staff meetings and in-services as requested.
