Execute protocol-driven chronic condition management. Perform skilled nursing procedures including wound care and medication administration (including vaccines). Provide comprehensive patient and caregiver education regarding chronic conditions and preventative care in person, over the phone, or in group settings. Conduct medication reviews within nurse-led visits to ensure patient safety and adherence. Conduct onsite testing (including point-of-care labs, EKGs, and specimen collection) and use patient data to identify and close gaps in preventative screenings, ensuring compliance with state scope of practice. Review and act upon high-priority abnormal laboratory results in a timely manner to maintain patient safety and continuity of care. Partner with providers and the interdisciplinary team to manage complex care plans and coordinate weekly transitions for patients moving from the hospital back to the clinic. Utilize standardized protocols for medication management, prescription renewals and prior authorizations. Ensures clinical communications are managed, tasks are completed and patient visit encounters are closed in a timely manner. Delegates to medical assistants and phlebotomists as needed to support patient flow and efficiency. Performs other related duties as assigned. Nurses on this unit care for patients with a variety of medical diagnoses or surgical patients with medical complications, which leads to excellent learning opportunities. The care environment on 8 Center allows nurses to develop exceptional clinical assessment, critical thinking, and problem solving skills while working collaboratively with the multidisciplinary care team to meet the needs of complex medical patients. 8 Center has an active Professional Nurse Council, and the staff nurses are able to be involved in unit decisions with a Shared Governance Model. Essential Functions Providing care to the patient according to the plan of care, specific unit routines and hospital policy under the Direction of the Registered Nurse. Documentation of care provided in the electronic medical record. Safe transport of patients via wheelchair or stretcher, dispatch services and appropriate documentation of services. Provides the services of an Observer for any patient on a patient care unit who has been assessed to require constant observation, either in person or via remote video monitoring, to maximize his/her safety. SUMMARY: The P articipant Care Associate (PCA), provides supportive services to the participant; including but not limited to in the home or community setting, during transport and on appointments, or and in the day health center. The PCA’s responsibilities for each participant are specified in the PCA/CNA Care Plan.
