Director of Nursing (DON) – Skilled Nursing Facility (SNF) Location: Elizabeth, New Jersey Job Type: Full-Time Position Summary We are seeking an experienced and compassionate Director of Nursing (DON) to lead and oversee all nursing operations at our Skilled Nursing Facility in Elizabeth, NJ. The Director of Nursing is responsible for ensuring the delivery of high-quality resident care, maintaining regulatory compliance, managing nursing staff, and fostering a culture of excellence and accountability. The ideal candidate is a strong clinical leader with a thorough understanding of long-term care regulations, staff development, quality improvement initiatives, and resident-centered care. Key Responsibilities Direct, supervise, and coordinate all nursing services within the facility. Ensure compliance with federal, state, and local regulations, including NJ Department of Health requirements. Develop and implement nursing policies, procedures, and standards of care. Recruit, train, mentor, and evaluate nursing staff, including RNs, LPNs, and CNAs. Oversee staffing schedules to ensure appropriate coverage and quality resident care. Monitor clinical outcomes and lead quality assurance and performance improvement (QAPI) initiatives. Collaborate with physicians, therapists, department heads, residents, and families to ensure comprehensive care planning. Manage infection prevention and control programs. Ensure accurate documentation and timely completion of clinical assessments. Participate in surveys, audits, and regulatory inspections. Maintain a positive work environment focused on teamwork, professionalism, and resident satisfaction. Qualifications Active and unrestricted Registered Nurse (RN) license in New Jersey. Minimum of 3–5 years of nursing leadership experience in a Skilled Nursing Facility, Long-Term Care, or Rehabilitation setting. Previous experience as a Director of Nursing, Assistant Director of Nursing (ADON), or equivalent leadership role preferred. Strong knowledge of CMS regulations, state survey processes, and long-term care operations. Demonstrated leadership, organizational, and communication skills. Ability to effectively manage staff, budgets, and clinical programs. CPR certification required. The Clinical Leader facilitates culturally sensitive patient care from pre-admission through post-discharge by managing, coordinating, and monitoring resources to achieve optimal clinical and financial outcomes, resource utilization, and quality service. This role assists the Nurse Manager in planning, directing, and supervising the overall operation of the assigned nursing unit on a 24-hour basis. Serving as a clinical resource and educator to unit and department staff in their area of expertise, the Clinical Leader works within the department and as part of CentraState’s management team to implement patient care and service goals established by senior leadership. Clinical decisions are grounded in evidence-based nursing practice to achieve desirable outcomes. Care is delivered using a patient-focused model supported by a multidisciplinary approach. Responsibilities Assists the Department Head in the continuing effort made by the department to improve the quality of patient care and services. Provides considerate respectful care focused upon the patients' individual needs. Affirms the patient's right to make decisions regarding his/her medical care, including the decision to discontinue treatment, to the extent permitted by law. Demonstrates the ability to obtain information and interpret information in terms of the needs of the age of the patients cared for. Takes immediate and appropriate actions in the event of a medication variance occurrence. Attends and participates in the multi-disciplinary discharge planning meetings and identifies the need for further services following discharge. Provides appropriate education based upon teaching plan. Conducts unit based education programs on a routine basis in collaboration with professional development. Meets standards for documentation regarding patient education. Documents results of patient assessment, nursing diagnosis, care plan nursing interventions and evaluation and patient progress in Medical Records, Nursing Standards of Care/Practice for the specific patient type. Recognizes changes in patient condition and/or emergencies situations and takes immediate and appropriate actions. Delegates components of care to non-professional caregivers as appropriate and evalutates outcomes. Makes decisions that demonstrate knowledge of nursing practice. Utilizes sound nursing judgment in clinical and patient care management situations. Demonstrates critical thinking skills in the care of patients and in the coordination of care to assure the achievement of maximum outcome. Develops new clinical pathways, monitors patient progress on current clinical pathways, reviews variances with Patient Care Reviewers, reviews variances from Quality Management with the Director/Nurse Manager and staff and reports to Nursing Quality Council. Follows the clinical progress of all patients. Participates in the development of unit protocols as needed. Conducts unit conferences to discuss quality issues and quality improvement strategies. Quality Referrals - unit based, follows up on nursing referrals, investigates problem, develops action plan, reviews with Nurse Manager, responds to Quality Management in writing, implements action plan. Follows up on patient complaints or concerns and keeps Director/Nurse Manager informed. Provides or coordinates at a minimum, 1 formal in-service per year to address unit specific needs. Monitors appropriate usage of speciality and Zonaire beds. Consistently adheres to hospital policy regarding the insertion, maintenance and removal of foley catheters, if applicable. Able to verbalize and demonstrate proper rationale and techniques surrounding all aspects of indwelling urinary catheter care as per policy # 4668. Fulfills the requirements for competencies and completion of annual mandatories surrounding indwelling urinary catheters. Takes a patient assignment when necessary. Qualifications Bachelor's degree in Nursing required. Master's degree in Nursing required or must be obtained within 3 years from date of hire (or internal transfer) as a condition of employment. A minimum of two years experience in ICU/CCU as an RN required. Previous supervisory experience preferred. Current NJ RN License required. BLS & ACLS Certification required. Certification in specialty area preferred. Completion of the specialty specific pharmacology test is required. Strong professional, organizational, and interpersonal skills to effectively relate with all members of the healthcare team.
