Department: MDS Reports To: Administrator Position Summary The Regional MDS Coordinator is responsible for overseeing MDS processes, PDPM optimization, and reimbursement integrity across assigned facilities. This role provides clinical, regulatory, and operational support to ensure accurate coding, timely submissions, and maximized reimbursement while maintaining compliance with all CMS guidelines. The position also serves as a key resource for education, auditing, and interim facility support. Key Responsibilities PDPM & Reimbursement Oversight Maintain and update the Regional PDPM Review Spreadsheet to ensure accurate tracking of all PDPM-related data. Essential Duties and Responsibilities: Administers medication and treatments as assigned within the scope of practice. Passes morning, afternoon and evening bedtime and overnight medications scheduled and as needed, staying with each resident until the medication is consumed, then charts appropriately. Maintains upkeep and cleanliness of medication carts and medication room, wipes down top of medication cart daily. Keeps the medication carts appropriately stocked. Responsible for recording and/ or restocking of medications from pharmacy no later than 7 days before medication runs out. Responsible for locking and storing narcotics (according to protocol) until turned over to pharmacy. Maintains accurate, complete and confidential resident medication and care records, according to established procedure and charts any unusual incident or information in resident Progress Notes and in staff Communication Log. Documents any and all changes in Communication Log along with giving a brief report to on-coming Medication Aide. Immediately report signs of resident changes to supervisor or Executive Director Fills out Incident Report for resident and/ or personnel when an incident occurs on shift. Takes and records vital signs as ordered and otherwise when necessary, i.e., monthly or daily as instructed by supervisor. Must be CNA Certified to do so. Notifies a supervisor at once if noting any adverse reaction of medication on resident or awareness of medication error. Observes and communicates to the Wellness Coordinator, Nurse/Executive Director any changes in resident physical or mental status. Assists with ADLs and assignment logs for residents whether they are on the assignment sheet or not and reporting any significant changes in residents or declines. Communicates, as needed, with physicians, families and other appropriate parties concerning resident status, changes in status, need for appointment, etc. Assist residents with personal hygiene and grooming twice a week, daily, or as needed. Under the mentorship of an assigned Cellular Therapy Nurse Coordinator II hereafter CTNC II, The CTNC I progressively increase performance to act independently in the utilization of excellent organization, multitasking, communication, and critical thinking skills to work within an assigned patient care team to coordinate the patient experience. This coordination includes the scheduling, accuracy, and follow up on all work up orders and procedures and follow up on all ancillary teams. The nurse will assist with patient and caregiver education, communicating with all stakeholders verbally, written and electronically. Working in clinic and performing ongoing assessments, nursing interventions, maintaining documentation and fee charges in accordance with organizational standards and regulatory guidelines and performing patient education. The candidate will strictly adhere to standard operating procedures, policies, REMs training guidelines, and the Foundation for Accreditation of Cellular Therapy guidelines.The activities of the CTNC include the coordination of care for autologous transplant patients which includes vital organ testing coordination and patient education. Coordinating insurance authorization for each of the phases of care. Scheduling of patients for cellular collection and arranging the appropriate admission date. The CTNC will also coordinate allogeneic transplants which include the above-mentioned coordination as well as working up a stem cell donor. The donor could be related and collected at Moffitt Cancer Center or be unrelated and collected by the national marrow donor program. Careful attention will be applied to ensure recipient and donor alignment. The CNTC will also be responsible for coordinating the care for patients requiring chimeric antigen receptor therapy or other immunotherapies. This process includes consulting with the patient, arranging for insurance authorization, vital organ testing, cellular collection, and admission coordination. This therapy also includes collaboration with outside pharmaceutical companies through their individual portals. This is done to arrange collection, manufacturing and delivery of cellular products while ensuring patient readiness for therapy.Activities also include astute understanding of individual insurance requirements and how that impacts coordination of the patient. Ability to utilize electronic calendars and EHR to coordinate patient care efficiently and effectively. Maintaining excellent communication with patients, their outside providers, and the whole medical team at Moffitt. Understanding of chemotherapy regimen and how that impacts the coordination process.
