Overview JOB SUMMARY: Responsible for the delivery of patient care through the nursing process of assessment, diagnosing, planning, implementation and evaluation, provided to the women and newborns during the antepartum, intrapartum, postpartum and neonatal stages of the childbirth process. The L&D RN is responsible for directing and coordinating all nursing care based on established clinical nursing care in the Labor and Delivery Department setting, including, but not limited to, providing labor support and education, monitoring maternal & fetal conditions, and monitoring the mother`s and baby`s vital signs during labor & birth, and managing a variety of potential issues that may arise. Collaborates with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes. The Labor and Delivery RN provides direct care, communicates with physicians / Nurse Director / Co – workers, as appropriate about changes in the patient’s clinical situation including results of diagnostic studies and is able to respond quickly and accurately to changes in situation or response to treatment. Responsibilities Consistently uses two patient identifiers when drawing lab, administering medications and performing procedures. Does not use unacceptable abbreviations in documentation, (using read back on all telephone orders.)Consistently follows medication reconciliation policy.Ensures Crash Cart, defibrillation function have been checked per protocol and after each code, appropriate items are immediately replaced and indicated by signature on crash cart checklist.Is in compliance with information contained in the Infection Prevention and Control Manual ( i.e. OSHA bloodborne pathogen standards, CDC Guidelines ), and demonstrates appropriate use of personal protective equipment ( i.e. gloves, gowns, masks, goggles ) and hand hygiene.Adheres to purposeful rounding on all patients as per hospital policy.Able to give thorough, detailed efficient change of shift bedside report to ensure consistency in the plan of care. Assists physicians with special tests and procedures in the unit. Uses time out on all patients for any bedside procedures.Avoids and prevents injury to self and patients by applying the principles of body mechanics. Maintains a safe environment, functioning with an awareness and application of safety issues identified within the unit/facility, i.e. suction and oxygen immediately available when needed.Consistent with pain assessment and reassessment with the time frame after giving pain medications and upon discharge or transfer of patients.Completes documentation of medications given to the patient on the medical record. Resolves any discrepancies in medication dispensing system.Recognizes signs of abuse and reports appropriately.Initiates discharge and patient / family education planning with proper documentation in the medical record.Establishes priorities of patient care based on essential needs and available resources of time, equipment, personnel and supplies.Formulates , maintains and updates a goal directed plan of care on patients which is prioritized and based on patient outcomes.Performs other duties as assigned or required. Performs all initiatives to promote exclusive breastfeeding and skin-to-skin. Qualifications EDUCATION, EXPERIENCE, TRAINING One year of experience in Couplet care or Labor and Delivery is required. Graduate of an accredited school of professional nursing certificate. Must have a current California RN license upon hire and maintained. BCLS, ACLS, NRP certifications through (AHA) is required upon hire. Basic Fetal Monitoring certificate upon hire, if no current Intermediate/ Advanced Fetal Monitoring course. Other certificates: Breastfeeding Training PT 1 & PT 2, and Intermediate/Advanced Fetal monitoring, within 6 months of hire, Ability to practice leadership, interpersonal skills, communication skills, teamwork, quality improvement and excellent customer service, and professionalism. Proficient in oral and written communication skills

Assist with bedside procedures, enforcing mandatory "time-out" safety protocols. Care Coordination & Handoffs: Conduct detailed bedside change-of-shift reports to ensure continuity of care, and participate in purposeful patient rounding. Infection Control & Unit Readiness: Maintain compliance with OSHA bloodborne pathogen standards, CDC infection control guidelines, and proper PPE protocols. Inspect emergency equipment (crash cart and defibrillator) per protocol. Job Summary: Works collaboratively with an assigned panel of physicians to manage the patients specialized needs. The managing team does differ according to the chronic disease. Duties include assessment to identify member needs and development of specific care management plan to address needs. In conjunction with the Physician, implements care/treatment plan by coordinating access to health services across multiple providers/ disciplines, monitors care, makes determination to arrange transportation and transfer patient if indicated, identifies cost-effective measures, makes recommendations for alternative levels of care and utilization of resources, promotes self-care management and ensures paper work is completed. Prepares reports, communicates program changes to appropriate staff and develops protocols in accordance with state regulations. Acts as a patient advocate and educator to assure that the patient has the knowledge to care for his/her condition and patient is educated and empowered to be responsible for participating in the plan of care. Develops individualized patient/family education plan focused on self-management, delivers patient/family education specific to a disease state. Develops and updates training and educational materials and presents to appropriate staff, members and families. Facilitates patients return to normal daily activities by teaching and making appropriate referrals for outside services/continued care. Consults with internal and external physicians, health care providers, discharge planners, and outside agencies regarding continued care/treatment or hospitalization or referral to support services or placement. May need to facilitate transportation and housing arrangements for patient. Coordinates transmission of clinical and benefit treatment to patients, families and outside agencies. Participates in data collection and analysis of clinical outcomes of care and customer satisfaction standards. Participates in the formulation and implementation/monitoring of action strategies and outcomes of care or customer service. Ensures that accurate records are maintained of the care associated with each patient. Interprets regulations, health plan benefits, policies, and procedures for members, physicians, medical office staff, and contract providers and outside agencies.
