RN (Registered Nurse) Job Duties: Assists in defining, maintaining and interpreting standards of practice. Performs required skills (assessment, intervention) according to guidelines, policies and procedures. Participates in interdisciplinary committees which influence or determine policies affecting nursing practice. Follow facility procedures/protocols for admitting residents transferring to hospitals/clinics/outpatient visits and completion of appropriate documentation based on established guideline. Makes rounds on all resident rooms and ensure that the L.P.N.’s, C.N.A.’s and/or others under his/her supervision receive their resident reports, has their assignments and break times scheduled. Assist the L.P.N. in establishing routines which facilitate a high level of nursing practice. Receives and give reports from and to outgoing and incoming shifts. Count or ensure the counting of controlled drugs with outgoing and incoming shifts. Coordinates/Prepares MDS schedules for quarterly, annual and significant change assessments. Complete MDS Booklet as per schedule (Nursing Sections). Reviews and signs off on all completed MDS and RAP Summary. Develops care plan that demonstrate knowledge of criteria to ascertain resident specific needs based on health care problem(s). Makes needed revisions and updates to the care plans. Ensures that care plan is carried out which demonstrates familiarity with criteria to establish priorities specific to anticipated outcomes. Monitors the provision of care for the residents in his/her assigned unit. Interprets philosophy and objectives of nursing service to residents, families and others. Identifies resident/family learning needs based on the discharge care plan as applicable and carries out appropriate teaching required. Demonstrates ability to coordinate and collaborate with resident/family and other members of the comprehensive health care team. Monitors the administration and accurate recording of medications used. Investigates and follow through on medication errors. Notifies physicians of pertinent observations and reactions, incident and abnormal laboratory and x-ray results as required and documents same promptly. Transcribes physician orders timely and ensure orders are implemented as appropriate. Reports all resident, visitor and work-related accidents/injuries and demonstrates appropriate follow through by investigating, documenting and processing incident reports. Accepts guidance, supervision, and criticism if needed. Practices in compliance with the Nurse Practice Act and regulatory agencies regulations. Assures that a stock level of medications, medical supplies, equipment, etc. are maintained at adequate levels at all times. Assume the authority, responsibility and accountability of directing your nursing shifts. Other responsibilities as deemed necessary and appropriate or as may be directed by the Director of Nursing Services or Administrator. 187602 Job Description Performs an age specific plan of care for a designated group of patients using the nursing process of assessment, diagnosis, outcome identification, planning, implementation and evaluation of patient care. Collaborates with physicians and other health team members in coordinating and implementing procedures and treatments. Uses leadership skills/clinical judgment in coordinating patient care and directing/delegating activities of the patient care unit team. Please note: The 'minimum’ rate and 'maximum’ rate refers to a 'per-visit' rate for regular assignments in this role. Voluntary work opportunities, outside the normal scope of duty, may pay an alternate ‘per-visit' rate, depending on multiple factors (region, type of visit, discipline, etc.), and may be offered on an occasional, as-needed basis. There is an hourly rate paid for certain administrative tasks (orientation, training, meetings, etc.), which is $30.00, following completion of new hire orientation. Job Responsibility Conducts pre-admission screening and initial nursing health assessment on new patients; assess body systems (cardiac, vascular, neurological, and gastrointestinal, etc.); develops nursing plan of care and goals based on patients needs. Provides nursing care to patients to include but not limited to supervising diet, medication, activity level of client, supervising safety in the home and activities of daily living. Evaluates the provision of nursing care to patients including but not limited to Evaluation of physical, therapy, occupational therapy, speech, therapy, nutrition counseling and respiratory therapy, Contracting agency nurses. Trains and evaluates home health aides, homemakers and personal care workers in the patient’s home, recommending personnel changes to the administration of Hospice as is necessary. Determines the need for, and recommends the rental or purchase of medical equipment and/or supplies for patients. Provides on-going evaluation of equipment for defects, repairs or additional needs. Recommends to the Hospice IDG (Interdisciplinary Group) when physical therapy, occupational therapy, speech therapy, nutritional counseling and respiratory therapy are warranted, as per his/her clinical assessment; requests physician written orders for changes in the plan of care as is appropriate; reports to and consults regarding patient status; participates in on-going orientation and in-service training offered both within and outside the Hospice. Participates in on-call coverage system on a rotating basis; serves as a liaison in maintaining the Hospice patients Plan of Care while inpatient; refers major or recurrent problems to the Hospice supervising nurse. Participates in the on-going Quality Improvement Program. Performs related duties as required.
