Key Responsibilities Obtains medical history and performs physical exam on patients. Description Kindred Hospital Clear Lake is a long-term acute care (LTAC) hospital specializing in the treatment of patients recovering from post-intensive care and medically complex conditions. Our hospital provides both intensive care and telemetry-level services, offering advanced monitoring and support for patients requiring extended medical care. With a multidisciplinary team dedicated to personalized treatment plans, we focus on helping patients achieve significant recovery and transition to the next level of care. Job Summary Provides quality patient care to critically ill patients, through the nursing process of Assessment, Planning, Intervention, and Evaluation. Serves as a patient advocate at all times. Supervises the nursing care provided to each patient by other caregivers. Develops the plan for nursing care as a result of coordination with the patient, family and staff of other disciplines as necessary. Applies critical thinking in order to promote positive clinical outcomes. Ensures patient education. Participates in discharge planning. Validates telemetry strips every 4 hours and PRN. Documents appropriately. Participates in assessing and meeting patient and family education needs. The Experienced Clinical Nurse at MD Anderson Cancer Center manages patient outcomes by providing, coordinating and supporting clinical aspects of direct patient care across the continuum of care. Summary The Division of Abdominal Transplant in the Michael E. DeBakey Department of Surgery is seeking a qualified Advanced Practice Provider (Physician Assistant or Nurse Practitioner) to join our multidisciplinary hepatology and liver transplant team. This faculty position offers the opportunity to practice in a nationally recognized transplant program providing comprehensive care to patients with chronic liver disease and undergoing liver transplantation. The APP will primarily provide outpatient hepatology care while collaborating closely with hepatologists, transplant surgeons, and other members of the hepatology and liver transplant teams. Job Purpose Successful candidates will be passionate about delivering exceptional patient-centered care, working in a collaborative academic environment, and contributing to the education of future healthcare professionals. Position Key Accountabilities: 1. Performs responsibilities in a manner that promotes quality patient care and customer service/satisfaction. 2. Effectively participates in planning and preparing for patient visits ensuring appropriate schedules, test results, and clinic specific requirements are available. 3.
