SUMMARY: The LPN is responsible for providing direct nursing care to the residents, and supervising the day-to-day nursing activities performed by nursing assistants. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required by the Director of Nursing to ensure that the highest degree of quality care is always maintained. ESSENTIAL DUTIES AND RESPONSIBILITIES: Direct the day-to-day functions of the nursing assistants. Meet with your assigned nursing staff, as well as support personnel, in planning the shift’s services, programs, and activities. Make written & oral reports/recommendations concerning the activities of the shift as required. Admit, transfer, and discharge residents as required. Provide leadership to nursing personnel assigned to your unit/shift. Complete & file required record-keeping forms/charts upon the resident’s admission, transfer, and/or discharge. Receive & transcribe telephone orders from physicians & record them on the Physician’s Order Form. Chart nurse’s notes in an informative & descriptive manner that reflects the care provided to the resident, as well as the resident’s response to the care. We are looking for a Licensed Practical Nurse to join our team and provide quality care to geriatric patients. The ideal candidate is a compassionate and experienced professional who is able to provide comprehensive nursing care, including administering medications, monitoring vital signs, and providing emotional support. The successful candidate will also have excellent communication skills and the ability to work in a fast-paced environment. Responsibilities: Administer medications and treatments as prescribed by a physician. Monitor vital signs, such as temperature, blood pressure, and pulse rate. Assist with activities of daily living (ADLs). Provide emotional support to patients and families. Educate patients and families about health care maintenance. Observe and report changes in patient's condition to the supervising nurse. Assist with geriatric care needs. Qualifications: Must be licensed as a Licensed Practical Nurse (LPN) by the state Must have no finding in the registry concerning abuse, neglect, or misappropriation of resident property. POSITION OVERVIEW The Memory Care Leader (CNA) is responsible for the daily functions of our Legacy Lane neighborhood, a memory care program, ensuring that dementia care practices are fully implemented and that they support the wellbeing of residents living with cognitive impairment. Key Responsibilities Collaborate with the Executive Director and leadership team to oversee the daily operations of the Legacy Lane programs on the health campus, ensuring alignment with dementia care best practices. Support efforts to maintain the appropriate resident population according to the company’s memory care move-in/move-out criteria.
