Enliven is seeking an experienced Registered Nurse to service home infusion and Infusion Center patients in Tyler, TX, and surrounding locations. The ideal candidate must be well-trained and able to give the best nursing care possible with little supervision. They will educate patients and their families on prevention and healthy habits. The ideal candidate must be compassionate, cool-tempered, and capable of handling emergent situations. They must follow health and safety guidelines faithfully and consistently. Responsibilities: Complete daily electronic forms for each assigned patient within a secured Electronic Medical Record (EMR) System. Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients. Administer ongoing care/infusions for each patient, and provide necessary follow-up as directed by the Physician and Clinical Manager. Confer with physicians and pharmacies in developing the initial plan of treatment based on the physician's orders and initial patient assessment. Provide hands-on care, management, and evaluation of the care plan and teaching of the patient under a physician based on ongoing assessments and as required by policy/regulation. Report patient care/conditions/progress to the patient's physician/pharmacy and Clinical Manager continuously. Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems. Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation. Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. What does the Nurse Manager (RN) do ? The Nurse Manager will work in conjunction with, and by the direction of, the Director of Nursing Services, if applicable, to provide nursing support for operations to existing practice locations and newly acquired practices by consulting and advising clinicians on best-practices, implementing of/providing education and training programs, and collaborating on interdisciplinary projects . Responsibilities The essential duties and responsibilities: Ensure clinical operations and practice consistent with organizational objectives Collaborates with practice management to provide support on operational projects such as site Quality Committees, Quality Oncology Practice Initiative site assessments, Advancing Care Information, the Oncology Care Model (OCM), and other special projects as assigned. Facilitates the implementation and/or utilization of outcomes data and evidence-based practices Facilitates implementation of standardized best practices within the organization. Ensures practice standards of care are consistent with community standards of care. Ensures the incorporation of clinical evidence into existing and newly developed standards. Staff Development Assess staffing levels and issues, patient flow, current practices in the clinical setting. Assists with the development of company-wide clinical competency testing and skills validation programs. Demonstrates working knowledge of applicable labor laws and schedules and coordinates employee staffing including vacations, sick leave and overtime. Other duties as assigned. Qualifications The ideal candidate for the Nurse Manager (RN) will have the following background and experience: Graduate of an accredited school of nursing, BSN or MSN preferred. RN state license and current registration with the State Board of Nursing in practicing state. Minimum 5 years actively practicing as a registered nurse. 5 years of leadership experience.
