They work closely with hospital leadership and transfer center teams to streamline patient transitions, reduce delays, and maintain continuity of care. The PLC-RN also monitors and identifies systemic barriers to patient throughput, analyzes trends, and reports findings to leadership to support strategic improvements in hospital operations. Qualifications Job Requirements: Graduate of an accredited school of Nursing. Current Multi-State Licensure as an RN. 3 years of clinical experience in an acute care setting.
