The RN will identify patient/family goals, develop a Plan of Care with patient, family, IDT, attending physician and/or partnering facility and continually evaluate progress towards goals. The Plan of Care will be updated according to policy/guidelines and as needs change (RN performs as case manager). The RN will demonstrate the ability to administer, monitor and document medications via all routes using age-specific care practices and provide appropriate patient/family teaching The RN will identify age-specific patient and caregiver learning needs, motivations, barriers, abilities and preferences and utilize teaching strategies/instructions and follow-through to match. The RN will follow palliative care and end of life principles for all interactions and provide compassionate care adhering to policy and guidelines. The RN directs and oversees the care provided by the LPN and Home Health Aide following agency policy and Care Center protocols.
