Job Description The Medical Coordinator (LVN) is responsible for delivering high quality medical care and influencing positive outcomes for patients experiencing homelessness in the recuperative care setting. We have many schedules available for this role, including weekends and nights. The pay range for this position is $34.00 - 36.00 per hour, depending on experience . This position also qualifies for $1.50 swing shift differential and $3.00 graveyard shift differential . The schedule for this position is 7am to 3:30pm, 5 days on, 2 days off. Sunday - Thursday. Work site may vary based on operational needs. Responsibilities Client Care: Perform initial intake to ensure that an appropriate care plan is established based on discharge information from the hospital or primary care notes Assess health status of patient upon arrival by conducting head to toe assessment and checking vitals Determine the patient’s insurance status, make follow up appointments with patient’s providers and reviewing medication list Establish and maintain a care plan Set weekly goals for patient to complete to assist with medical or social reintegration, verify goal completion, and assess medical needs. Overview The staff RN is a professional caregiver who assumes responsibility and accountability for a group of patients for a designated time frame and provides care to these patients via therapeutic use of self, the nursing process, the therapeutic environments, treatment and modalities and interdisciplinary collaboration. Responsibilities The RN shall, for their patient, collaborate in the formulation of the overall plan of care for the patient, as evidenced by written documentation. Identifies needs and problems specific to biophysical, cognitive, developmental, emotional, socio-cultural, and spiritual. Demonstrates accountability for on-going awareness of the status of assigned patients. Collects and documents data appropriate to the nature and severity of the condition. Qualifications Minimum 2 years experience in Labor & Delivery. Current CA Registered Nurse license. Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), Neonatal Resuscitation Program (NRP) and Advanced Fetal Monitoring certifications are required. NIHSS certification is required within 90 days of employment.
