About the Opportunity As a Home Care Field RN / Outpatient Nurse , you will travel to client homes and community-based settings across assigned counties, conducting in-home or community-based nursing assessments, and providing high-quality care to patients in their homes and communities. Responsibilities: Conduct comprehensive psychiatric evaluations and assessments of elderly patients in nursing homes. Diagnose and develop individualized treatment plans based on the patient's mental health needs. Provide medication management and adjust treatment plans as necessary.
