Job Summary/Overall Objectives The RN assesses, plans, implements and evaluates the care for a designated group of patients. The RN coordinates, directs, and actively participates as an effective member of the interdisciplinary health care team, contributing to the efficient operation of the unit and promoting positive patient outcomes. Essential Job Functions Maintain professional and technical knowledge by attending education workshops, review professional publications, establish personal networks, and participate in professional societies. Protect patients and employees by adhering to infection-control policies and protocols, medication administration and storage procedures, and controlled substance regulations. Document patient care services by charting in patient and department records. Maintain continuity among nursing teams by documenting and communicating actions, irregularities, and continuing needs using Nurse Knowledge Exchange techniques. Maintain patient confidence and protects operations by keeping information confidential. Identify patient care requirements by establishing personal rapport with potential and actual patients, and other persons in a position to understand care requirements. Establish a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families. Promote patient's independence by establishing patient care goals, teaching patient/family to understand condition, medications, and self-care skills, and answering questions. Maintain safe and clean working environment by complying with procedures, rules and regulations; calling for assistance from health care support personnel. Resolve patient problems and needs by utilizing multidisciplinary team strategies. Maintain a cooperative relationship among health care teams by communicating information, responding to requests, building rapport, and participating in team continuous quality improvement and problem-solving methods. Contribute to team effort by accomplishing related results as needed. Ensure operation of equipment by completing preventive maintenance requirements by following manufacturer's instructions, troubleshooting malfunctions, calling for repairs, maintaining equipment inventories, and evaluating new equipment and techniques. Maintain nursing supplies inventory by checking stock to determine inventory level, anticipating needed supplies, placing and expediting orders for supplies, verifying receipt of supplies, and using equipment and supplies as needed to accomplish job results. The Registered Nurse may provide direct oversight of the GN, LPN, Technical Partner, ALS Technician, Nurse Extern and Unit Clerks. This oversight includes the Registered Nurse conducting the initial comprehensive assessment and interpreting the assessment to initiate the interprofessional plan of care. This oversight includes support of the delegation of duties that are within the scope and capability of the care team. Coordinates communications between patients/families and physician practice, pharmacies, specialists, and insurance carriers. Provides advice based on protocols and physician instructions and directs patients to the appropriate level of care. Facilitates patient's access to prescription refills based on physician order. Obtains prior authorizations and/or assists with indigent programs per physician order based on the patient's financial status or insurance coverage. Completes medication reconciliation according to policy Provide continuity of care for patients in collaboration with physicians, with accountability for planning according to Pathways and policies/procedures. Evaluates adherence to the Clinical Pathway for assigned patient group in collaboration with physician. Demonstrates competence in patient and family education including, but not limited to, etiology, comprehensive wound care, dressings, edema control, specialty foot care and life style changes. Adheres to all policies/procedures for HBOT patient care/safety Assists HBOT technician with patient care as needed. Completes HBOT training course if deemed necessary for center operations. RN HBO trained, will accomplish all supportive tasks/duties assigned and deemed to be safe by the program safety coordinator (when applicable). Attends IHEAL training Enters patient assessment and visit information accurately and according to policy Completes Level of Care accurately Other related duties as assigned. Competency and Experience [Knowledge, Skills and Abilities] Required Qualifications/Experience Graduate of an accredited Nursing Program required. One (1) year Med/Surg Experience Strong leadership ability, good organizational skills, independent and critical thinking skills, sound judgment, and knowledge of legal aspects and liability of nursing practice. Strong ability to communicate complex and/or controversial topics and concepts to a wide and diverse audience. Preferred Qualifications/Experience Bachelor’s Degree in Nursing preferred. Previous experience in an outpatient office setting. National Alliance of Wound Care and Ostomy Certification Board (NAWCCB): credential is Wound Care Certified (WCC). Wound Ostomy Continence Nursing Certification Board (WOCNCB): credential Certified Wound Care Nurse (CWCN). American Board of Wound Management (ABWM): credential Certified Wound Specialist (CWS). License, Certification & Clearances Current licensure to practice as a Registered Nurse in the State of Pennsylvania required. Current Basic Life Support (BLS) approved curriculum of the American Heart Association (CPR and AED) program required. Act 33 with renewal. Act 34 with renewal. Act 73 FBI Clearance with renewal. The Licensed Practical Nurse functions as a member of the health-care team by exercising sound nursing judgment. The LPN provides direct and indirect patient care for an assigned group of patients under the direction and supervision of the RN and according to the plan of care derived from physician's orders and Registered Nurse assessment. Nursing Care includes physical care, emotional support, and patient and family education. Delivers direct and indirect patient care in accordance with the Pennsylvania Practical Nurse Law, hospital policies, procedures, protocols and other professional standards of care. a. Assists the RN in collecting patient health care data on admission by completing a focused assessment in an ongoing systematic manner, focusing on the physical, psychosocial, age, cultural and spiritual needs of the patient and which contributes to the comprehensive assessment by the RN. b. Works directly with the RN to implement an individualized, age-specific plan of care and evaluate its effectiveness. c. Assists the RN in reassessment of the patient's condition and takes appropriate action as indicated or by unit specific policy. d. Involves the patient/family and other health care providers when appropriate. e. Completes patient care related activities within scheduled time periods. f. Complies with completeness, accuracy and timeliness of documentation, utilizing the electronic medical record. 3. Observes, records and reports to the appropriate professional the symptoms, reaction and changes including: a. General physical and mental condition of patients, and signs and symptoms which may be indicative of untoward changes; and stresses in human relationships between patients, between patients and personnel, and between patients and their families and visitors. 4. Performs nursing functions as defined in Pennsylvania regulations, such as: a. Administration of medications, immunizations, and therapeutic treatments prescribed by the physician; b. Initiates (peripheral short catheter) and maintains IV therapy; c. Maintenance, monitoring and discontinuance of blood, blood components and plasma volume expanders; d. Maintenance and discontinuance of IV medications and fluids via a patient-controlled administration system; e. Administration, maintenance and discontinuance of parenteral nutrition; f. Collection of blood specimens from an IV access device; g. Preparation and care of patients receiving specialized treatments administered by the physician or the professional nurse. 5. Assists with the rehabilitation of patients according to the medical care plan, being aware of and encouraging the interests and special aptitudes of patients. a. Encouraging patients to help themselves within their own capabilities in performing activities of daily living. b. Knowledge and application of the principles of prevention of deformities, the normal range of motion, body mechanics, and body alignment. 6. Assists in providing education for the provision and coordination of patient care activities that promote and maintain health, foster self-care and improve outcomes. a. Assists the RN in assessing the patient's readiness to learn, identifying educational barriers and adapting the process to meet the needs of the patient. b. Ensures education about safely and effectively using medication, pain and managing pain, available resources, how to obtain further care, and information about responsibilities in their care. c. Acquires and distributes educational resources tailored to patient needs. d. Provides and reviews, upon request of the RN, written discharge instructions that demonstrate an inter-relationship between education, discharge planning and continuity of care. 7. Maintains a collaborative relationship with physicians and other health care providers to achieve desired patient outcomes and assure a continuous flow of patient care. a. Communicates pertinent information for interdisciplinary patient care conferences, shift reports and internal and external transfers. b. Reports changes in patient's condition to the RN, including the need to reassess patient or change the plan of care; confers with the RN when there is a need to contact the physician. 8. Participates in the unit performance improvement activities designed to enhance the quality of patient care and customer service. a. Assists in data collection process as requested. b. Recommends modifications for performance improvement. 9. Demonstrates leadership by utilizing the concepts inherent to the practice of the professional nurse. a. Demonstrates an ability to solve problems independently and seeks assistance from immediate supervisor when appropriate. b. Delegates patient care appropriately. c.
