Position Summary: Functions as a member of collaborative interdisciplinary team providing comprehensive care to patients and families with complex healthcare needs that may be both predictable and unpredictable. Position Key Accountabilities: Performs responsibilities in a manner that promotes quality patient care and customer service/satisfaction. Adheres to broad scope of practice as established by Nurse Practice Act. Ensures the efficient operation of the clinic or unit by proper scheduling and planning for optimum use of staff and resources, maintenance of stock and supplies, and efficient and safe use of equipment. Uses systematic approach to provide nursing care to patients and families/populations by performing focused and comprehensive assessments through subjective and evidence based objective data. Prioritizes, synthesizes and documents assessments and nursing plan of care using critical thinking and judgment. Implements plan of care within scope of practice through assignment and monitoring of other nurses and delegation and monitoring of unlicensed staff. Initiates standing orders for prescription refills and labs as well as standing delegation orders where appropriate. Communicates effectively with collaborative team while escalating critical results and issues in an appropriate and timely manner. Exercises judgment while coordinating care and referring patients to agencies or health care professionals for other services/transitions in care where indicated. Performs onsite and telephone triage using best practice guidelines and documentation to ensure patients receive the right level of care in the right setting at the right time by the right caregiver using the right equipment and supplies. Acts as a patient and healthcare advocate by maintaining a safe environment and promoting quality healthcare and services. Respects and maintains privacy and confidentiality per HIPAA guidelines. Adheres to standards of practice for professional nursing. Acts as a clinical and leadership resource for team members. Develops and implements teaching plans concerning maintenance, promotion and restoration of health. Evaluates patients' responses and outcomes to education and therapeutic interventions. Home Infusion Registered Nurse (RN)

Job Summary Enliven is seeking an experienced RN (Registered Nurse) to service home infusion and Infusion Center patients in Spring and surrounding locations. The ideal candidate must be well-trained and able to give the best nursing care possible with little supervision. They will educate patients and their families on prevention and healthy habits. The ideal candidate must be compassionate, cool-tempered, and capable of handling emergent situations. They must follow health and safety guidelines faithfully and consistently. The goal is to promote patients' well-being by providing high-quality nursing care. Responsibilities: RN (Registered Nurse) Complete daily electronic forms for each assigned patient within a secure Electronic Medical Record (EMR) System. Provide admission, case management, and follow-up skilled nursing visits for home infusion and infusion center patients. Administer ongoing care/infusions for each patient, and provide necessary follow-up as directed by the Physician and Clinical Manager. Confer with physicians and pharmacies to develop the initial treatment plan based on the physician's orders and the initial patient assessment. Provide hands-on care, management, and evaluation of the care plan and teaching of the patient under a physician based on ongoing assessments and as required by policy/regulation. Report patient care, conditions, and progress to the patient's physician, pharmacy, and Clinical Manager continuously. Implement patient care plan in conjunction with patient and family to assist them in achieving optimal resolution of needs/problems. Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation. Prepare appropriate medical documentation on all patients, including case conferences, patient contacts, medication order changes, re-certifications, progress updates, and care plan changes. Prepare visit/shift reports, update/summarize patient records, and confer with other healthcare disciplines to provide optimal patient care.
