Accurately and completely rooms patients, takes and documents vital signs, prepares patients for provider exam Follows clinic protocols and directions given by nursing team leader or supervisor. Performs diagnostic testing including but not limited to EKGs, audiograms, tympanograms, spirometry, and visual screenings with reporting and documentation as appropriate. Monitors inventory of medical and/or office supplies and equipment, ensuring units and/or rooms are properly stocked and supplies are current and not expired. Orders supplies as needed. Responsible for reviewing both Provider refill in basket and assisting with daily medication refill request using the ARC refill protocol. Responsible for reviewing EPIC triage call in basket for any callbacks. Cleans patient rooms as needed, including proper handling of dirty laundry. Ensures laundry cart is stocked with fresh linens on a daily basis. May assist providers with sigmoidoscopy exams and cardiac stress testing by setting up equipment, assisting with patient preparation, and cleaning and storing equipment after exam. Reports and documents appropriately. Maintains equipment (autoclaves, sterilizes, washes speculums, checks expiration dates, etc.) Adheres to all company policies, including but not limited to, OSHA, HIPAA, compliance and Code of Conduct. Regular and dependable attendance. Follows the core competencies set forth by the Company, which are available for review on CMSweb. Works holiday shift(s) as required by Company policy. LVN: Administers medications and injections. Provides direct nursing care to patients and distributes medications and treatments under physician order according to accepted standards of care. Teaches and counsels patients and families concerning heath assessment and care including prevention of disease and health maintenance. May provide vaccine management including ordering and monitoring vaccine usage/wastage, facilitating the maintenance of the vaccines, participating in educational opportunities, utilizing the Texas Vaccine for Children Provider Manual, and completing reportable documentation as recommended by Texas Department of State Health Services. May facilitate the implementation, monitoring and evaluation of the immunization program. Calls patients and informs of lab results. Accurately and completely reports and documents status, care rendered, response to care, provider orders, contacts with patient and/or other healthcare providers. Accurately and completely performs patient follow-up activities including laboratory results and diagnostics, appointments, referrals, medications, and call backs. Participates in the orientation of new employees. Staff Medical Assistant: Provides patients with educational materials as directed. Administers medication under physician’s direction and within the scope of practice as demonstrated by completion of training and testing. Responsible for reviewing result note in basket and calling /informing patients of normal lab results. Reviews provider instructions regarding abnormal results. QUALIFICATIONS: LVN: Education: High school diploma or equivalent. Graduation from an accredited school of vocational nursing. ALL this must be documented in 24hr report. Re-order medications, check medication for expiration dates, if expired removed from med cart and place in Discontinue box in medication room Refresh Ice during your shift in the dietary department. (Water pitcher dated with current date) Organize medication storage room 3 x weeks. Assigned days will be posted. Responsibilities: Essential Duties: • Accurately interview patient and record patient history and measure and record vital signs such as height, weight, BMI, head circumference, current medications, pulse oximetry level and patient comfort level. • Verify patient information such as, but not limited to demographics, insurance, vitals and medications; complete intake process including verifying and entering patient data into EHR/EPM. • Perform duties such as greeting and assisting patients in a courteous and professional manner, answer telephone and provide information as necessary, verify patient information, etc. • Collect, perform, document, and report results of CLIA waived lab tests including, but not limited to: urine dip, pregnancy tests, finger stick blood glucose levels, fecal occult blood tests, rapid influenza tests, and rapid strep tests. • Collect, prepare and document specimen collection including, but not limited to: blood, urine, stool, and sputum. • Perform clinical duties as assigned including, but not limited to: preparing patients for examinations and /or treatments, assisting provider with exams or procedures, setting up exam or procedure rooms, and preparing instruments for sterilization. • Complete pre visit planning to support the clinical visit. • Administer medications per CUC formulary. • Perform medical acts as prescribed by Standing Delegation Orders. • Dismiss patient at end of clinic visit including completing the orders documented in the orders module, tasks, and patient plan. • Participate in orientation of new employees and attend mandatory meetings. • Work as a team member, interacting with staff, patients and providers to communicate and coordinate patient care and work flow effectively. • Coordinate and manage patient care including communicating test results to patient, assist with pre-certifications, referrals, authorizations, prescriptions and scheduling of patients for clinic, hospital and other ancillary medical treatments and tests. Follow up on diagnostic test to ensure that test have been completed and documentation is completed in EHR. • Perform surveillance programs to include but not limited to complete medical records including labs, authorizations, X-Rays, hospital records and any previous care pertaining to each patient visit • Maintain a clean, functional environment including cleaning and disinfection of equipment, exam rooms and storage areas as per CUC guidelines • As assigned, may serve as a scribe accurately and thoroughly document medical visits and procedures as they are being performed by the provider, including but not limited to the following duties: o Patient medical history and physical exam, o Procedures and treatments performed by healthcare professionals, including nurses and physician assistants. o Patient education and explanations of risks and benefits. o Provider-dictated diagnoses, prescriptions and instructions for patient or family members for self-care and follow-up o Prepare referral letters as directed by the provider • Perform other duties as assigned. Qualifications: MINIMUM EDUCATION: • Graduation from high school or equivalent. • Medical Assistant Diploma and/or National Medical Assistant Certification verifying graduation from a Medical Assistant Program or equivalent military healthcare experience. MINIMUM EXPERIENCE: • Completion of a medical assistant externship REQUIRED CERTIFICATIONS/LICENSURE: Current Health Care Provider Cardiopulmonary Resuscitation (CPR) certification through American Heart Association or American Red Cross.
