PURPOSE AND SCOPE: The professional registered nurse Inpatient Services RN CAP 1 is an entry level designation into the Clinical Advancement Program (CAP). This position is accountable and responsible for the provision and coordination of clinically competent care including assessment, planning, intervention and evaluation for an assigned group of patients. This may include delegation of appropriate tasks to direct patient care staff including but not limited to RNs, LVN/LPNs and Patient Care Technicians. As a member of the kidney disease health care team, this position participates in decision-making, teaching, leadership functions, and quality improvement activities that enhance patient care outcomes and program operations. PRINCIPAL DUTIES AND RESPONSIBILITIES: All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care, policy, procedures, standards of nursing practice, applicable contractual service agreements, state and federal regulations. · Performs all essential functions under the direction of the Supervisor and with guidance from the Educator, Preceptor or in collaboration with another Registered Nurse. · Performs ongoing, systematic collection and analysis of patient data pre – during – post treatment for assigned patients and documents in the patient medical record, makes adjustments and modifications to treatment plan as indicated and notifies Supervisor, Physician, patient’s primary nurse and others as may be indicated. · Assesses, collaborates and documents patient/family’s basic learning needs to provide initial and ongoing education to patients and family. · Recognizes aspects and implications of patient status that vary from normal and reports to or collaborates with appropriate health team members for input including but not limited to Supervisor, appropriate physician, and the contracted facility primary Nurse. · Directs and provides safe effective patient care for assigned patients as prescribed for all modality specific treatment procedures. · Assesses daily assigned patient care needs and collaborates with direct and ancillary patient care staff as needed. · Initiates and coordinates communication with FKC and Non-FKC dialysis providers and appropriate contracted facility personnel as needed to provide continuity of patient care. · Performs the implementation, administration, monitoring, and documentation of patient's response to prescribed intradialytic transfusions, including appropriate notification of adverse reactions to physician and appropriate blood supplier. · Administers medications as prescribed and documents appropriate medical justification and effectiveness. · Initiates and assists with emergency response measures. · Serves as a resource for health care team, participates in staff training and orientation of new staff as assigned. · May be asked to provide specialized nursing care instruction to hospital/facility staff as stipulated contractually. · May be assigned to assist in an Outpatient facility on an as needed basis. · Required to complete CAP requirements to advance. · Performs all other duties as assigned by Supervisor. SUPERVISION: Assigned oversight of Patient Care Technicians/LVN/LPNs/RNs as a designated Nurse in Charge, after meeting all the following: · Successful completion of all FKC education and modality specific training requirements for new employees. · Must have a minimum of 9 months experience as a RN. · 6 months experience in acute dialysis as a RN. EDUCATION and LICENSURE: · Graduate of an accredited School of Nursing. · Current appropriate state licensure. · Current or successful completion of CPR BLS Certification · Must meet the practice requirements in all states in which he or she is employed. EXPERIENCE AND REQUIRED SKILLS: · Entry level for RNs with less than 2 years of Nephrology Nursing experience as an RN · Minimum 9 months experience as a Registered Nurse, 12 months (preferred). · 6 months acute dialysis experience (preferred) · Hemodialysis and/or ICU experience (preferred). JOB DESCRIPTION SUMMARY The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Provides health care instructions to the patient as appropriate per assessment and plan. 10. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. Job Title/Position: Registered Nurse 11. Additional Duties 1. Participates in on-call duties as defined by the on-call policy. 2. Ensures that arrangements for equipment and other necessary items and services are available. 3. Supervises ancillary personnel and delegates responsibilities when required. 4. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and inservice classes. 5. Fulfills the obligation of requested and/or accepted case assignments. 6. Actively participates in quality assessment performance improvement teams and activities. You will work in community group homes in the Beaver County area. You will provide oversight of medical care for 4-6 individuals per home during your shift. You will be required to travel between homes as needed. You will ensure physician orders, medications and treatments are carried out as well as perform general health monitoring. Care Comfort Hospice is looking for Part Time/PRN LPN Hospice Nurses. ALL SHIFTS AVAILABLE! FLEXIBLE SCHEDULE! I. Qualifications Skilled Nurse from an Accredited program currently licensed and in good standing with the Pennsylvania State Board of Nursing. One to 2 years’ experience in nursing in an acute care setting or 1-year experience in home health, hospice or comparable experience is preferred. Desire to learn and demonstration of aptitude for the hospice philosophy and comfort care required. II. Responsibilities It is the responsibility of the Visit Nurse to coordinate and provide the highest level of hospice care to patients and families. The Visit Nurse works in conjunction with the Nurse Case Manager, Director of Nursing (DON) and the Interdisciplinary Team to: · provide hands-on clinical nursing, interacting with patients, family, physicians, Interdisciplinary Team members, and other health care professionals involved in the patient’s care. · assess and coordinate a pain/symptom management program formulated with the participation of family and patient, attending physician and the Interdisciplinary Team, utilizing pharmacological and non-pharmacological methods of pain/symptom control to enhance quality of life. · assist in facilitating freedom of choice, relief of suffering and death with dignity for patients facing end of life issues. · provide support and education for families based on the hospice philosophy of care. Care includes physical, psychological, spiritual, and financial comfort. · participate in Interdisciplinary Team meetings and up-dates the Plan of Care (POC) for assigned patients based on identified problems and needs. · ensure all services provided are in accordance with the patient’s POC. · ensure the POC involves the active participation of the patient, family, PCP, and the Interdisciplinary Team · provide Nurse Case Manager with appropriate information to coordinate all services related to hospice care for assigned patients. Coordination of services include communicating and making referrals for home health aide/homemaker, chaplain, social worker, volunteer, and ancillary services, as well as ordering medications and Durable Medical Equipment · educate patient and family on the disease process and expectation related to the disease progression. · assist with CQI activities. · work as a team member at all times and uphold the Hospice Code of Ethics · provide care to manage patient symptoms. · utilize the medication formulary whenever possible for the management of patient symptoms. · reports to DON as patient condition changes, changes in the patient condition that require change in level of care. III. Orientation and Training Care Comfort Hospice will provide orientation and training, as necessary, to assist the Hospice Nurse in performing the responsibilities listed above. Training may be in-house or outside seminars. Evidence of orientation and training will become part of the personnel file. IV. On Call The Hospice Nurse will be on call according to scheduling. During the hours when on call, the Visit Nurse is expected to follow On Call Protocol and to respond to all pages within 20 minutes. V. Accountability Reports directly to the Nurse Case Manager and Director of Nursing.
