Description The Offsite Medical Assistant I Provides clinical office support to Physicians & Surgeons and performs patient care and administrative duties necessary to deliver quality patient care. Responsibilities 1. Fulfills patient care responsibilities as assigned which may include: checking schedules and organizing patient flow; accompanying patients to exam/procedure room, assisting patients as needed with walking, transferring, dressing, collecting and processing specimens and point of care testing, preparing for exam, etc. Assists physicians with various procedures, takes vital signs, relays instructions to patients and or families; answers calls and provides pertinent information. 2. Fulfills clerical responsibilities as assigned which may include: sending and receiving patient medical records; obtaining lab/x-ray reports, hospital notes, referral information, etc. Completing forms and requisitions as needed, managing charts to ensure information is completed and filed appropriately. These responsibilities may be carried out using the electronic medical record (EMR) system or manually as necessary. 3. Enters orders into the electronic medical record (EMR) system or prepares prescription refill requests on behalf of the physician for the review and approval by the ordering physician. 4. Position Overview The Registered Nurse (RN) Supervisor plays a key leadership role in managing nursing operations during assigned shifts at Constant Care Agency’s partner nursing facilities in the Bronx. The RN Supervisor ensures that residents receive safe, high-quality care consistent with the standards of practice, regulatory requirements, and the mission of Constant Care Agency. This position requires strong clinical judgment, supervisory ability, and compassion for the elderly and long-term care population. Primary Responsibilities Clinical Supervision Oversee all nursing and direct care staff (RNs, LPNs, CNAs) on assigned units or shifts. Ensure timely and accurate delivery of nursing care in accordance with physicians’ orders, facility policies, and resident care plans. Monitor residents’ conditions; respond promptly to changes in status and initiate appropriate interventions or physician notifications. Conduct walking rounds at the beginning and end of each shift; verify continuity of care and accurate reporting. Oversee medication administration and treatments to ensure safety and compliance. Staff Leadership & Management Assign, direct, and evaluate the performance of nursing staff; delegate tasks appropriately based on scope of practice. Provide mentorship, education, and on-the-job training for new or less experienced staff. Ensure adequate staffing levels; manage call-ins and replace staff as needed in collaboration with the staffing coordinator. Promote teamwork, professionalism, and accountability across the care team. Regulatory Compliance Ensure nursing practices comply with New York State DOH , CMS , and facility policies . Conduct chart audits to confirm accurate, complete documentation of resident care and progress notes. Participate in QA/QI programs, infection control audits, and safety committees. Prepare for and participate in DOH inspections and surveys. Maintain resident confidentiality and HIPAA compliance at all times. Emergency Response Serve as the on-site clinical authority during assigned shifts. Respond to medical emergencies, perform immediate assessments, and direct staff response. Communicate critical incidents to the DON or Administrator on call. Resident & Family Relations Foster compassionate, resident-centered care and ensure residents’ dignity and rights are upheld. Communicate effectively with residents and families regarding care issues or changes in condition. Participate in care plan meetings when required. Qualifications Licensure: Current, unencumbered Registered Nurse (RN) license in New York State . Experience: Minimum 2–3 years of RN experience , with at least 1 year in a long-term care or skilled nursing facility . Prior supervisory experience strongly preferred. Education: Graduate of an accredited nursing program (BSN preferred). Certifications: BLS required; ACLS preferred. Knowledge: Comprehensive understanding of NYS DOH and CMS nursing home regulations. Familiarity with MDS documentation, infection control, fall prevention, and pressure injury protocols. Skills: Strong clinical assessment and problem-solving ability. Excellent leadership, delegation, and interpersonal communication skills. Ability to handle emergencies calmly and professionally. Proficiency in electronic health records (e.g., PointClickCare, SigmaCare). Work Conditions Work performed in a skilled nursing facility environment. Requires standing and walking for extended periods and occasional lifting up to 25 lbs. May require evening, night, weekend, or holiday shifts. Overview Provides high quality comprehensive nursing care to meet the needs of the patient/client and manages/coordinates the delivery of cost effective multidisciplinary health care services for a caseload of patients/clients consistent with VNS Health philosophy, policy, goals and objectives, and Standards of Nursing Practice. Works under moderate supervision. For Nurse-Family Partnership (NFP) only: Provides health education, social support and community linkages to meet the needs of pregnant and postpartum, first-time mothers and their infants. Also responsible for adhering to NFP national guidelines. • Assesses the physical, social and psychological needs and environmental conditions of the patient/client through professional knowledge, skills of observation, interviewing, and physical examination to develop a plan of care. • Understands, interprets, and implements reimbursement protocols; determines the billing source for patient/client care. Consults with management on complex/unique reimbursement cases. • Acts as the coordinator of care for the multidisciplinary team by providing and managing the care needs of patients/clients which includes making/confirming referrals to other disciplines/community agencies and ordering supplies/services and ensuring they are delivered as needed. • Documents content and process of all services provided in accordance with government regulations/requirements, and VNS Health policies, practices, and procedures, as well as NFP requirements as appropriate. Initiates and maintains written (including clinical and progress notes) and verbal communications with team members and management to assure optimal patient/client care. • Participates in the orientation of selected staff through ‘buddy system’ field observation of staff and feedback to appropriate staff involved in orientation process. • Participates in selected student programs by providing guidance, training and mentoring to students. • Provides nursing care in accordance with VNS Health policies, practices, procedures and Standards of Nursing Practice, which may require standing, stooping, sitting, crouching, bending and stretching to deliver patient/client care. • Transports and utilizes required medical equipment and other supplies using VNS Health designated/supplied carrying case weighing approximately 25 – 30 lbs. to and from patient/client homes/care facilities. • Travels to patient/clients’ homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient/client. • Follows work procedures in relation to established workflows. • Maintains productivity sufficient to meet VNS Health goals. • Assumes responsibility for personal professional growth through attendance at in-services, training, continued formal education and select journal readership. • Performs other related duties and participates in special project, as assigned. • For CHHA and Hospice Only • Establishes, implements, and revises a plan of care with input from appropriate multidisciplinary health team members (i.e., physician, OT, PT, SW, SLP, family members, caregivers, HHAs, etc.) based on goals mutually acceptable to the patient and significant others. Ensures that services follow the plan of care by ongoing assessment, planning, coordination, counseling, teaching and supervision of the delivery of care to meet the needs of the patient. • Oversees/supervises the care provided to the patient by Licensed Practical Nurses and Paraprofessionals through regular home visits in compliance with regulatory and VNS Health requirements. Assesses the patient’s condition, monitors the effect of the care plans on the patient/family health status, informs physician and other team members of changes in the client's needs and updates the plan of treatment, as needed. • Manages, plans, coordinates and executes the patient’s safe discharge from service. Ensures that physician’s orders, discharge summary and communication to team members, patient and family members adhere to VNS Health and regulatory requirements. • Communicates and consults with patient’s MD and follows-up on problematic situations affecting care such as abnormal clinical findings, medication discrepancies, or other factors that impact patient’s ability to comply with the plan of care. Manages and coordinates all aspects of assigned caseload. • Assists patients with activities of daily living, which may require positioning, moving, transferring and lifting patients of varying weights and physical conditions, with and without assistance and as appropriate, from family members and/or paraprofessionals. • For NFP Only • Establishes, implements, and revises a plan of care with input from the client and from appropriate multidisciplinary health team members (i.e., social worker, physician) based on goals mutually acceptable to the client. Ensures that services follow the plan of care by ongoing assessment, planning, coordination, counseling, teaching, and evaluation of the delivery of care to meet the needs of the client. • Manages, plans, coordinates and executes the client’s safe discharge from service. Ensures that discharge summary and communication to team members, patient and family members adhere to VNS Health and regulatory requirements. • Maintains a flexible work schedule to meet clients needs. • Participates in all required NFP training sessions. • Participates in special projects and performs other duties as assigned. Qualifications Licenses and Certifications: License and current registration to practice as a registered professional nurse in New York State required Valid driver's license or NYS Non-Driver photo ID card, may be required as determined by operational/regional needs. For Hospice Only: Wound Care Certification required or willingness to obtain within one (1) year of hire preferred Certification in Hospice and Palliative Care Nursing preferred Education: Degree in Nursing from an approved program required Bachelor's Degree NFP Only- Nursing required Work Experience: Experience for CHHA: Minimum of one year of experience in a medical/surgical environment required Experience for Hospice: Minimum of one year of clinical nursing experience, including at least six months of Hospice, or Med/Surg or ICU or Emergency Dep’t or Home care, Palliative Care, Oncology or Long Term Care experience required Experience for NFP: Minimum of one year of recent experience in maternal/child health required Demonstrated basic computer skills required Pay Range per Visit: If you are applying to the per diem per visit version of this job, the hiring range is as follows: $78.64- $123.20 per visit.
