Implement clinical and Telehealth protocols as outlined CMO and Supervising Physicians. Analyze and interpret patients' histories, symptoms, physical findings, and diagnostic information to develop appropriate diagnoses. Deliver excellent patient care through in-patient and virtual diagnostic and therapeutic recommendations with attention to patient-centric care, safety, cost, and reliably accurate information. Ensure that the activities of the Provider are conducted in a manner that is consistent with overall department expectations and are in compliance with Federal and State regulations, guidelines, and requirements including working knowledge of all health information management issues such as HIPAA. Ability to work efficiently in a fast-paced, autonomous environment within both in-patient clinic and virtual settings. Ability to pursue queue of telehealth patients in a timely fashion to ensure proper patient follow up. Order, perform, or interpret the results of diagnostic tests, as well as responsibly prescribe medications and educate patients on continued treatment and care of acute and chronic conditions. Attest and follow clinical practice guidelines by the Office of Medical Affairs. Attend Mandatory monthly meetings with Supervising physicians and E/M training. Ability to perform responsibilities within standard NP/PA protocols. Responsible for learning the aspects of compliance in the company by completing all mandatory compliance training in order to meet and exceed our continued quality of care. DEA required for all providers; however, where limited by years of practice under state law, DEA will be required within 120 days of provider meeting the minimum years of practice under state law for obtaining DEA. Providers must be able to treat all ages and must meet any credentialing requirements needed; and DOT certification is required to be obtained by FT and PT Providers within 120 days of employment and maintained during employment. Job Summary Provides planning and delivery of direct and indirect patient care through the nursing process of Assessment, Planning, Intervention, and Evaluation. The Seasonal Staffing program provides a temporary opportunity for the employee to work a minimum of 36 hours (typically 3–12-hour shifts) per week for a 12-week commitment period at a Seasonal Agreement rate of pay. Once the initial commitment period has exhausted the employee can extend the agreement up to an additional 12 weeks in 4-week increments. The employee’s status during this period will be Per Diem and ineligible for benefits. At the end of the Seasonal Agreement the employee may has the option, based on current job openings, to change status to full-time, part-time, or per diem. Administers medication utilizing the five rights of medication administration reducing the potential for medication errors. Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning; family is included in teaching as appropriate. Interacts professionally with patient/family and involves patient/family in the formation of the plan of care. Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic and religious/spiritual needs of patients and their families. Functions as liaison between administration, patients, physicians, and other healthcare providers. Interprets data about the patient’s status in order to identify each patient’s age specific needs and provide care needed by the patient group. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Manages and operates equipment safely and correctly. Position will be 7 days on 7 days off We are seeking a dedicated Afterhours RN Care Manager to provide high-quality, compassionate hospice care outside standard business hours. This role ensures seamless coverage, continuity of care, and strong clinical management for patients and families during nights, weekends, and on-call hours. About the Role The Afterhours Care Manager RN is responsible for assessing and coordinating patient and family needs during after-hours shifts and maintaining primary clinical oversight for assigned patients. This includes routine visits, admissions, death visits, urgent after-hours calls, and any visit assigned per the patient’s plan of care. This role serves as an active member of the Interdisciplinary Group (IDG) to ensure coordination between physicians, families, and the hospice team. Clinical administrative responsibilities, workflow auditing, and survey readiness support are also part of this role. Essential Functions Ensure appropriate delivery of hospice services to eligible patients and families. Provide direct in-person patient care during on-call, weekend, and after-hours shifts, including assessments, admissions, routine visits, death visits, and live discharges. Conduct initial and ongoing assessments of physical, emotional, psychosocial, and spiritual needs. Support the IDG process through chart reviews, plan of care updates, and interdisciplinary communication. Develop and revise the Hospice Plan of Care in collaboration with physicians and IDG members. Educate patients and families on disease progression, symptom management, medications, and hospice philosophy. Communicate significant changes in patient condition promptly to hospice and attending physicians. Maintain accurate, timely clinical documentation including assessments, care plans, and notes. Assist with rescheduled, missed, or reassigned visit requests based on triage direction. Participate in calendar and scheduling modifications, including service codes and clinician assignments. Support administrative and compliance activities including workflow audits, survey readiness tasks, and documentation collection. Assist with TPE reviews, QAPI efforts, and regulatory compliance. Ensure infection control standards and universal precautions are consistently followed. Provide leadership and guidance to field staff; conduct supervisory visits when appropriate. Participate in required meetings, after-hours updates, and trainings. Maintain proficiency in pain and symptom management for hospice and end-of-life care.
