The Clinical Nurse Coordinator serves as a vital clinical link for pediatric patients with complex cardiac, pulmonary, genetic, and multisystem conditions requiring specialized evaluation and long-term management of pulmonary hypertension. This position builds long-term relationships with patients and families, serving as a consistent point of contact throughout the entire continuum of care across inpatient, outpatient, procedural, pharmacy, home-health, and community settings. The Clinical Nurse Coordinator prepares patients for clinics and coordinating diagnostic testing like echoes, labs, and cardiac catheterizations.
