Purpose of Position: Under the direction of the Paraprofessional Team Leader the Certified Home Health Aide (CHHA) performs assigned duties in the patient’s home in a safe and considerate manner as prescribed by the case manager. Description HOME HEALTH AIDE TRAINEE PRIMARY DUTIES AND RESPONSIBILITIES: Attend all HHA certification course education and clinical trainings. Successfully complete HHA course and certification examination. Maintains open communication with case manager and/or Team Leader/Assistant Team Leader regarding health, safety and environment status of patients. Reports all changes in a timely manner. Complies with State/Federal regulatory acts. Perform all skills with competency, as indicated on the skills checklist. Communicates with Clinical Coordinator regarding all scheduling concerns relating to patients which may include, but not limited to changes such as times, days, patient absences and discharges. Communicates with case manager, and if not available, the Team Leader/Assistant Team Leader regarding any case concerns, patient problems, planned time off or other general schedule changes. Completes and submits required documentation, i.e., patient and employee time sheets, etc., accurately and on a timely basis. Report all concerns of possible patient abuse or neglect to the case manager and/or the Team Leader. Establishes and maintains a therapeutic relationship with patients. Perform only those tasks as prescribed by the case manager on the home health aide plan of care. Encourage patient in achieving an optimum level of independence by encouraging his/her participation in personal care and in interests and activities in the home, family and community. Provides or assists with post mortem care after pronouncement. Perform additional tasks as may be assigned by the Team Leader and/or case manager, depending on the specific situation or on the Home Health Aide’s experience and capabilities. Example: routine uncomplicated colostomy case and decubitus care. Provides understanding and emotional support to patient and significant others. If eligible, may participate in the Patient Transportation program for the Private Services department. All other duties as assigned. Qualifications Home Health Trainee Education/Experience/Training High School Diploma or equivalent preferred. Ability to read, write and communicate in English. Purpose of Position: The NP/PA will work with the Inpatient Cardiovascular Medicine Service within the Cardiovascular Division of the Department of Medicine. This NP/PA will assist the attending cardiologists in all areas of patient care in the Cardiovascular Medicine Service. Rotation and call schedules will be determined by patient need and may include both nighttime on-call responsibilities and weekend coverage. Description Cardiology Service responsibilities: Will provide Cardiology services as required for patients at the Hospital, all without regard to source of payment. All such Services shall be performed consistent with all applicable legal requirements and in a professional and cooperative manner. Assist in the planning, implementation, and evaluation of Cardiology and related services provided at the Hospital. Responsible for obtaining and coordinating the following general items: history and physical, laboratory results, charting of clinical findings, admissions orders, nursing instructions and other required elements. Purpose of Position: To provide direct nursing care to the residents, patients, and to supervise the day-to-day nursing activities performed by the nursing assistants. Such supervision must be in accordance with current federal, state and local standards, guideline and regulations that govern our facility, ensuring that the highest degree of quality care is maintained at all times. Readies equipment and aids physician during treatments and examinations of patients. Observes patient, records significant conditions and reactions, and notifies supervisor or physician of patient's condition and reaction to drugs, treatments, and significant incidents. Takes temperature, pulse, blood pressure, and other vital signs to detect deviations from normal and determines progress of patient. May serve as leader for group of personnel rendering nursing care to number of patients. Must maintain the confidentiality of all resident care information including protected health information.
