Our fully staffed anesthesia team is expanding coverage opportunities across both Meritus Medical Center and our growing ASC network. Whether you want consistent supplemental work or occasional shifts around another position or personal schedule, our PRN model is designed to provide flexibility. There is no minimum monthly shift requirement to join or remain part of our PRN team. Care Continuum Navigator | Part Time 32 hrs. biweekly, 11am - 730pm, Weekend Option Available The Care Continuum Navigator (Registered Nurse) plays a vital role in ensuring patients receive coordinated, patient-centered care across the healthcare continuum. Working collaboratively with the Emergency Department (ED), Inpatient Care Management, physicians, primary care providers, post-acute care facilities, and community organizations, this role focuses on improving outcomes for high-risk patients by addressing medical, behavioral, and social determinants of health. The Care Continuum Navigator identifies patients with frequent or potentially avoidable emergency department visits and hospitalizations, assesses the underlying causes driving healthcare utilization, and develops individualized care plans that promote health, independence, and appropriate use of healthcare resources. This position serves as a patient advocate and care coordinator, helping patients and their caregivers navigate complex healthcare systems while connecting them with the services and support needed to achieve their health goals. Success in this role is measured through improved patient outcomes, reduced avoidable hospital utilization, enhanced care coordination, and achievement of established quality performance metrics. Key Responsibilities Collaborate with Emergency Department and Inpatient Care Management teams, physicians, primary care providers, skilled nursing and assisted living facilities, and community agencies to coordinate seamless transitions of care. Experience - ASN with a minimum 3 years acute care experience and/or combined relevant experience in acute care Utilization role, SNF, or community health setting, or BSN with at least 2 years of acute care experience and/or combined relevant experience in acute care, utilization role, SNF, or community health setting. Licensure/Certification - Current license to practice as Registered Nurse in the State of Maryland, or if resident of a state that participates in Nurse Multi-State Compact agreement, holds Registered Nurse License from that state that is designated as "Compact" or "Multi-State Privilege" using primary source verification and provides additional documentation as required by the Maryland Board of Nursing. Overview Meritus Health is building a high-autonomy house-calls program for patients who need wound care and longitudinal primary care in their homes and in our Home Health clinic. We’re looking for a Nurse Practitioner who wants to own their panel, practice at the top of license, and be the clinical backbone of this model. Role Summary In this role, you will provide in-home visits for wound care and primary care, with the potential for some clinic-based sessions at our Home Health department location. You will manage a defined panel of patients, coordinate with Home Health nurses and physicians, and have significant day-to-day independence in how you structure your schedule and care plans. Key Responsibilities Provide comprehensive in-home primary care: history, physical exam, diagnosis, and ongoing management of chronic and acute conditions. Deliver wound care in the home and clinic, including assessment, debridement (within scope), dressing selection, and application of advanced therapies. Develop and own individualized care plans for your patient panel, with an emphasis on preventing avoidable ED visits and hospitalizations. Perform periodic clinic sessions at the Home Health location for follow-up, new consults, and procedures best done in a clinic setting. Coordinate closely with Home Health nurses, PT/OT, and referring physicians to ensure seamless transitions and aligned treatment plans. Order and interpret labs, imaging, and other diagnostics; adjust treatment based on results and patient goals. Prescribe medications, wound therapies, and durable medical equipment within scope and state regulations. Educate patients and caregivers on wound care, chronic disease management, medication adherence, and preventive health. Document promptly and accurately in the EMR, including clear care plans and communication with the broader care team. Participate in case reviews/huddles with the Home Health and medical teams to review panel outcomes and refine protocols. What We’re Looking For Licensed Nurse Practitioner (Family or Adult-Gerontology) in Maryland, with prescriptive authority (or eligibility). Minimum 2–3 years of experience in primary care, home-based care, wound care, or similar high-acuity outpatient setting. Comfort working independently in the field while collaborating tightly with an interdisciplinary team. Valid driver’s license and reliable transportation for house calls within our service area. How This Role Is Structured You will own a panel of patients attributed to you, with defined visit frequency based on clinical need and program protocols.
