OVERVIEW: Under the direction of the Director of Nursing, supervises the nursing personnel and the day-to-day nursing activities of the facility during an assigned tour of duty. Such supervision must be in accordance with accepted professional standards and current federal, state and local regulations to ensure the highest degree of quality care is always maintained. RESPONSIBILITIES: · Monitor the daily delivery of nursing care and nursing staff performance as they deliver nursing care to the patients/residents in accordance with established policies and procedures · Plan and conduct shift staff meetings on a regular basis to assist in identifying and correcting problem areas and to assist in the improvement of services · Identify problems or potential problems in the delivery of nursing care to residents and implement corrective action immediately · Make written and oral reports/recommendations to the Director of Nursing as necessary · Assist in developing methods for coordinating nursing services with other resident services to ensure the continuity of the resident's total regimen of care · Begin investigation of all accidents, including staff interviews, data gathering and report writing · Interpret the nursing department's policies and procedures to personnel, resident, visitors and government agencies as required · Ensure staff always maintain confidentiality of resident information · Develop work assignments and/or assist in completing and performing such tasks · Delegate to each Charge Nurse the administrative authority, responsibility and accountability necessary to perform his/her assigned duties · Make daily rounds of assigned units at a minimum at the beginning and end of the shift, depending upon the activity of the units · Follow-up on complaints and grievances filed by resident, families or staff. Take corrective action as needed and make reports to the Director of Nursing · Report to Director of Nursing all reports of suspected abuse · Ensure that emergency equipment/supply checks are done each shift · Ensure that the resident's environment is maintained in a clean and safe manner · May make determination of death when necessary · Participate in the orientation program for new assigned nursing staff in order to facilitate their assimilation into the work place within a reasonable time span and to ensure that each staff member understands his/her role, responsibilities and standards of performance · Complete written performance evaluations on the assigned staff following probation and thereafter annually, and when performance dictates the need, complete additional evaluations · Initiate corrective and progressive disciplinary action, as needed · Ensure that nursing personnel are performing their work assignments in accordance with acceptable nursing standards · Ensure that all nursing service personnel comply with the policies and procedures, including dress code as set forth in the Nursing Procedure Manual · Ensure that a enough nursing staff are scheduled and available for your/next tour of duty to ensure that quality care is maintained, and nursing care needs of the patient/resident are met · Monitor time and attendance of nursing personnel and take corrective action as appropriate · Make recommendations to the Director of Nursing regarding employee dismissals, transfers etc. · Identify learning needs of the nursing staff · Plan, schedule and conduct unit-based in-services as needs are identified · Monitor assigned personnel to ensure that they are following established procedures in the use of equipment · Recommend to the Director of Nursing the equipment and supply needs of the department · Monitor nursing procedures to ensure that nursing service supplies are used in an efficient manner to avoid waste · Ensure the prompt repair or replacement of medical/nursing equipment when necessary · Oversee the nursing care to patients/residents to ensure safe, efficient and customer-oriented services are delivered at all times. · Inform nursing staff of new admission arrivals and the expected time of arrival · Ensure that rooms are ready for the new admission · Visit resident on report daily in order to observe and evaluate each resident's physical and emotional status · Review the nurse's notes to ensure that they are informative and descriptive of the nursing care being provided · Provide direct nursing care as necessary · Start and restart IV’s when ordered and when necessary · Ensure the staff refer to the resident's care plan prior to administering care to the resident · Assist the staff nurses in monitoring seriously ill resident · Ensure that all nurses on your shift comply with the written procedures for the administration, storage and control of medications and supplies · Monitor medication passes and treatments to ensure compliance with physician orders and facility policy · Review medication administration records (MAR) for completeness of information, accuracy in the transcription of physician orders · Inspect nursing treatment areas daily, such as clean utility room, to ensure that they are maintained in a clean and safe manner · Meet with family members and resident as needed. · Consult with the resident, family and physician in planning the resident's care, treatment and rehabilitation · Assist in arranging transportation for transfers to the hospital · Participate in the implementation of facility quality improvement program for the nursing service department · Participate in facility surveys (inspections) made by authorized government agencies as requested by the Administrator or Director of Nursing · Attend and actively participate in various committee meetings such as Infection Control, Safety, and Performance Improvement as requested · Implement recommendations from committees as instructed by the Director of Nursing · Perform Performance Improvement audits as assigned · Participate in and adhere to employee health, safety, security and corporate compliance programs and activities · Participate in the implementation and maintenance of the Infection Control program for monitoring and/or reporting communicable and/or infectious diseases among resident and personnel · Assist in developing safety standards for the nursing service department as necessary · Ensure that personnel follow the established procedures for the use and disposal of personal protective equipment · Report malfunction of equipment and/or broken furniture immediately to Maintenance Department · Receive annual physical including appropriate screening for tuberculosis · The ability to conduct all activities within the resident abuse prevention manual · Cooperate with investigations regarding employee health, safety, security, and corporate compliance programs · Report all accident and incidents to Supervisor immediately upon occurrence · Correct unsafe or hazardous condition encountered, if unable to correct, report condition to immediate Supervisor upon discovery · Conduct all activities within established safety, security and infection control procedures and guidelines Conducts all activities within established corporate compliance policies and procedures · Observe and respond to all standing security precautions, devices and alerts as they pertain to work and common area · Respond to fire/disaster drills and emergencies · Ensure residents safety in accordance with resident safety program. · Follow safety features of the plan of care · Ensure that all personnel performing tasks that involve potential exposure have attended the appropriate in-service training prior to performing such tasks · Ensure that the assigned nursing personnel follow established infection control procedures when isolation precautions become necessary · Ensure that personnel follow the established procedures for the use and disposal of personal protective equipment · Ensure that all residents are wearing appropriate identification bracelets · Participates in mandatory in-service and job training · Assist the Director of Staff Development in providing off shift in-services as necessary · Assist in the Quality Assurance and Performance Improvement (QAPI) process QUALIFICATIONS: EDUCATION / EXPERIENCE: · Bachelor of Science in Nursing (preferred) · Current registration and license to practice as a Registered Nurse in the state employed in · Peripheral IV certification · Two years of clinical nursing experience · One-year experience in a management position preferred · One-year experience in geriatrics and subacute OVERVIEW: Responsible for the overall operation and optimal quality of care for the residents on the assigned unit. Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the facility's administration and the Nursing Policies and Procedures. As an LPN you will be responsible for providing residents assistance with ADLs and in the administration of their medications while overseeing staff. Administrative Functions · Plan, develop, organize, implement, evaluate, and direct the nursing service department, as well as its programs and activities, in accordance with current rules, regulations, and guidelines that govern the nursing care facilities. · Develop, maintain, and periodically update written policies and procedures that govern the day-to-day functions of the nursing service department. · Maintain a reference library of written nursing material (i.e., PDR’S, Regulations, Standards of Practice, etc.) that will assist the nursing service department in meeting the day-to-day needs of the resident. · Make recommendations for developing, maintaining and periodically updating the Employee Handbook to the Administrator. · Develop and implement a nursing service organization structure. · Make written and oral reports and recommendations to the Administrator concerning the operation of the nursing service department. · Develop methods for coordination of nursing services with other resident services to ensure the continuity of the residents’ total regimen of care. · Develop, implement, and maintain an ongoing quality assurance program for the nursing service department. · Participate in Facility surveys (inspections) made by authorized government agencies, as necessary or as may be directed. · Assist in developing and implementing appropriate plans of action to correct identified deficiencies. · Assist the Discharge Planning Coordinator in developing, implementing and periodically updating the written procedures for the Discharge Planning Program. · Assist the resident and Discharge Planning Coordinator in planning the nursing services portion of the resident’s discharge plan. · Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary. · Monitor the Facility’s QI, QM, and survey reports. Personnel Functions · Inform the Nurse Supervisor and/or Unit Manager of staffing needs when assigned personnel fail to report to work. Recommend overtime for assigned CNAs and other nursing personnel to the Nurse Supervisor or Unit Manager to insure residents’ needs are met. · Report absentee call-ins to the Nurse Supervisor and/or Unit Manager. · Provide feedback and recommendations for probationary and annual employee performance evaluations for CNAs and other nursing personnel. Prepare probationary and annual employee performance evaluations for CNAs and other nursing personnel. Meet with CNAs and other nursing personnel to review performance. · Develop work assignments and/or assist in completing and performing such assignments. Reassign assigned CNAs and other nursing personnel based upon skills and qualifications. · Provide leadership, education, counseling, and discipline, when appropriate, to assigned CNAs and other nursing personnel. · Make daily rounds of your unit/shift to ensure that assigned CNAs and other nursing personnel are performing their work assignments in accordance with acceptable nursing standards. Make changes to assignments based upon resident needs and availability of skilled staff. · Evaluate daily performance of assigned CNAs and other nursing personnel. Document any disciplinary issues and report problem areas or disciplinary actions to the Nurse Supervisor and/or Unit Manager. · Meet with your assigned CNAs and other nursing personnel, on a regularly scheduled basis, to assist in identifying and correcting problem areas, and/or to improve services. Assign personnel accordingly to insure quality resident care while allowing personnel to improve in areas where they may be deficient, with appropriate supervision. · Ensure that CNAs, department personnel residents, and visitors follow the department’s established policies and procedures at all times. · Resolve personnel disputes and issues within the unit and reassign or remove personnel to insure patient safety. · Develop and maintain a good working rapport with inter-departmental personnel, as well as other departments within the Facility to ensure that nursing services and activities can be adequately maintained to meet the needs of the residents. · Review complaints and grievances made or filed by your assigned CNAs or other nursing personnel. Make appropriate reports to the Nurse Supervisor and/or Unit Manager as required or as may be necessary. Follow Facility’s established procedures. · Receive or provide the nursing report upon reporting in and ending shift duty hours. · Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the Facility’s policies and procedures governing accidents and incidents. Committee Functions · Serve on, participate in, and attend various committees of the Facility as appointed by the Administrator. · Evaluate and implement recommendations from established committees as they may pertain to nursing services. · Assist the Pharmaceutical Services Committee in developing, maintaining, implementing, and periodically updating written policies and procedures for the administration, storage, and control of medications and supplies. · Schedule, attend, and participate in developing the agenda for Care Plan Committee meetings as necessary. · Participate in regularly scheduled reviews of the Discharge Plans and make such plans available to the Care Plan Committee, as required or as may be necessary. Personnel Functions · Determine the staffing needs of the nursing service department necessary to meet the total nursing needs of the residents. · Supervise and assist scheduling of employees within established state guidelines. · Recommend the number and level of nursing personnel to be employed. · Assist the Administrator and/or the HR Director in the recruitment and selection of nursing service personnel. · Ensure that all nursing assistants are enrolled in or have graduated from an approved training program. · Assign a sufficient number of LPNs and RNs for each tour of duty to ensure that quality care is maintained. · Assign a sufficient number of CNAs as applicable, for each tour of duty to ensure that routine nursing care is provided to meet the daily nursing care needs of each resident. · Ensure that all nurse aide trainees are under the direct supervision of a licensed nurse. · Develop work assignments and schedule duty hours, and/or assist nursing supervisory staff in completing and performing such tasks. · Delegate to nursing service supervisory personnel the administrative authority, responsibility, and accountability necessary to perform their assigned duties. · Complete and/or delegate the completion of all annual reviews for the nursing services department. · Monitor absenteeism to ensure that an adequate number of nursing care personnel are on duty at all times. · Develop, maintain, and periodically update the written procedure for ensuring that professional nursing personnel, including private duty nurses, have valid and current licenses as required by this State. · Review complaints and grievances made or filed by department personnel. · Provide complaint and grievance reports to the Administrator, as required or as may be necessary. · Ensure that departmental disciplinary action is administered fairly and without regard to race, color, creed, national origin, age, sex, religion, handicap, or marital status in accordance with all applicable state laws. · Assist the Infection Control Coordinator in establishing a TB management program for employees. · Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the Facility’s policies and procedures governing accidents and incidents. · Ensure background checks on nursing service personnel in accordance with established procedures. · Ensure that all CNAs as applicable, credentials are verified through the State Nurse Aide Registry. · Ensure that appropriate adverse personnel actions relative to CNAs as applicable, employment criteria are reported to the State Nurse Aide Registry. · Terminate employment of personnel when necessary, documenting and coordinating such actions with the HR Delegate and/or Administrator. Develops nursing care plan of assigned patient on admission, updates plan of care as needed and ensures plan of care is coordinated with patient, family, and other members of the team. Assesses and reassesses pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management. Revises the plan of care as indicated by the patient’s response to treatment and evaluates overall plan daily for effectiveness. Performs patient care responsibilities considering needs specific to the standard of care for patient’s age. Receives physician’s orders, ensures transcription is accurate and documents completion. Administers medication utilizing the five rights of medication administration reducing the potential for medication errors. Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning; family is included in teaching as appropriate. Assists physicians with examinations, treatments and special procedures and performs services requiring technical and manual skills within scope of practice. Performs treatments and provides services to level of licensure. Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of patients and their families. Functions as liaison between administration, patients, physicians, and other healthcare providers. Interacts professionally with patient/family and involves patient/family in the formation of the plan of care. Interprets data about the patient’s status to identify each patient’s age specific needs and provide care needed by the patient group. Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors. Initiates or assists with emergency measures for sudden adverse developments in patients’ condition. Answers telephone, paging system, patients’ call lights, anticipates patients’ needs, and makes rounds of assigned patients and responds as appropriate. Consults other departments as appropriate to provide for an interdisciplinary approach to the patient’s needs. Provides end of shift report to oncoming nurse, narcotics are counted, documentation is complete, and physician orders signed off. Communicates appropriately and clearly to management, co-workers, and physicians. Identifies and addresses psychosocial needs of patients and family; communicates with Social Service/Discharge Planner regarding both in hospital and post discharge needs. Participates in orientation, instruction/training of new personnel. Manages and operates equipment safely and correctly. Knowledge/Skills/Abilities/Expectations Knowledge of medications and their correct administration based on age of the patient and their clinical condition. Basic computer knowledge. Able to organize tasks, develop action plans, set priorities and function under stressful situations. Qualifications Education Postsecondary certificate, diploma, or program graduation from an accredited school of nursing. (Required) Associate’s Degree or Bachelor’s Degree in Nursing.
