Licensed Practical Nurse Full Time Position Available Pay: $20.40 Welcome Bonus: $3000 This is a fantastic role to continue your nursing career path and work within a setting that offers variety and rewarding challenges. You will have large impact by providing nursing services, training, and supports to address the medical or psychiatric needs of individuals with a brain injury, intellectual and developmental disability, a substance use disorder, and/or has an emotional or behavioral disability. Monitor the health conditions of individuals served and ensure quality services are provided. Administer medications and provide documentation as well as oversee and monitor staff members who administer medications. Provide prescribed medical treatment by physician’s orders and personal care services. Conduct assessments, implement health care services to be included in each Individual Service Plan (ISP), and respond to changes of condition. Provide training to staff members including universal precautions, infection control, and medication administration. Serve as liaison and advocate with other medical and professional service providers. Provide training for individuals served with topics including self-medication, prescribed treatments, sex education/STD prevention and smoking cessation. Qualifications: Associate’s Degree in Nursing or related field, or a graduate of an accredited LPN program. One year of experience working with population served in a nursing capacity. Current state LPN or LVN License. Current driver's license, car registration, and auto insurance. Additional certifications as required by state licensing regulations such as CPR/FA and HIV/AIDS certification, BBP, and ACT training. Ability to effectively communicate with staff, individuals served, and families. Exhibits the MHS Standards of Excellence and exercises strict confidentiality at all times. Job Functions: Participates in bedside shift report/handoff. Performs hourly rounding. Obtains required patient assessments and reassessments per nursing policy. Develops a plan of care and teaching plan based on assessed needs of patient and family. Revises as needed per hospital policy. Coordinates timely, adequate teaching to patient/family/significant other in preparation for discharge and documents appropriately. Implements and monitors nursing interventions. Consistently documents assessments, interventions, and outcomes in Meditech within required timeframe. Participates in interdisciplinary rounding and collaborates with team members to identify patient needs. Recognizes changes in patient conditions and communicates with physician using SBAR. Effectively manages patient care and delegates appropriately according to the needs of the patient and the skill of the caregiver. Utilizes appropriate interpersonal communication which facilitates productive relationships with other team members. Facilitates a positive and caring relationship with others by utilizing trust and mutual respect. Establishes a therapeutic relationship between patient, family and self, based on empathy and trust. Promotes a positive patient experience. Seeks help from appropriate resources as needed. Maintains current knowledge of and practices within the scope of hospital policies. Minimum Education/Experience Required: Current licensure in the State of Ohio or if working in WV will need a WV license as a Registered Nurse. BLS required upon hire. ACLS within 1 year of employment required. Special Knowledge, Skills, Training: Verbal, clerical, numerical, and manual dexterity, color vision and near visual acuity, proper lifting techniques and body mechanics.
