Scope Of Position UConn Community Health Network Medical Group is seeking a compassionate, clinically skilled, and patient-focused Medical Coordinator – LPN to support patients and clinical teams through telephone triage, care coordination, and clinical support services. Scope Of Position UConn Community Health Network Medical Group is seeking a compassionate, clinically skilled, and patient-focused Medical Coordinator – LPN to support patients and clinical teams through telephone triage, care coordination, and clinical support services. The Medical Coordinator – LPN serves as an important point of contact for patients, assessing their needs, providing appropriate clinical guidance, and determining the appropriate level and timing of care.
