Key Responsibilities Supervise, mentor, and support clinical staff across home care, assisted living, and skilled nursing facility settings. Monitor and ensure compliance with state and federal regulations (including Medicare hospice benefit requirements, documentation, licensure) as well as internal policies and accreditation standards. Oversee staff scheduling, oncall rotation, visit coverage, and staffing models to maintain patient care continuity. Collaborate with the admissions/referral team to ensure smooth clinical onboarding of new patients and transitions of care. Review clinical documentation, patient outcomes, and quality metrics; identify and implement process improvements. Provide leadership in symptom management, endoflife care best practices, patient/family education, and staff education/training. Act as a clinical resource for staff, patients and families — supporting complex clinical cases, ethical issues, and family communication. Participate in budgeting and resource planning for clinical operations as needed; monitor supply usage and equipment needs. Qualifications Education & Licensure: Registered Nurse (RN) with active Connecticut license. Bachelor’s Degree in Nursing (BSN) preferred; relevant master’s degree or leadership preparation a plus. Experience: Minimum of 35 years of hospice or palliative care nursing experience (home care or facility-based) required. Prior supervisory or leadership experience in a clinical setting preferred. Familiarity with hospice regulatory requirements, clinical documentation, IDT processes, and interdisciplinary care. Skills & Competencies: Strong leadership, coaching and mentoring skills — able to develop and support a highperforming clinical team. Excellent interpersonal and communication abilities — able to work with patients, families, clinical staff, and leadership. Proven ability to manage complex clinical operations, prioritize multiple tasks, handle change and work in a fastpaced environment. JOB DESCRIPTION SUMMARY The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Patient Care 1. Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: A. Writing and initiating plan of care B. Regularly re-evaluating patient and family/caregiver needs C. Participating in revising the plan of care as necessary 4. Initiates the plan of care and makes necessary revisions as patient status and needs change. 5. Uses health assessment data to determine nursing diagnosis. 6. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 7. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 8. Counsels the patient and family in meeting nursing and related needs. 9. Provides health care instructions to the patient as appropriate per assessment and plan. 10. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. 11. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Summary: The Nursing Supervisor assists the Program Manager in running the day-to-day operations of the Withdrawal Management and Intensive units. This role includes supervising nursing and technician staff, managing daily operations including the admissions & discharge process, census reporting, DMHAS reporting, unit schedules, and all aspects of client care. The Nursing Supervisor is responsible for ensuring appropriate nursing and technician coverage across all shifts and provides on-call support and unit coverage as needed, including outside of regular business hours. Essential Duties &/ Responsibilities: Provides administrative supervision to all nurses & technicians in alignment with the supervision hours required by ASAM. Assists program manager in completing annual evaluations for all nurses & technicians. Provides on-call support to the unit as scheduled. Responsible for completing payroll timely and correctly. Provides training and orientation to the assigned team of nurses & technicians, including onboarding and training coordination to new staff. Assists in hiring and recruiting process, including facilitating interviews. Manages the nursing & technician schedule and always ensures appropriate coverage. Oversees census reporting for InterCommunity and DMHAS. Develops and leads in-service trainings with the entire team and leads program improvement initiatives. Assists with nurse & technician duties on the unit and in triage. Provides nursing support to nurses, including medication administration, admissions, assessments, and care coordination. Coordinates treatment with other providers to support Whole Person Healthcare. Takes a lead in managing crisis situations on the unit. Responsible for maintaining safety and cleanliness of the unit by timely submission of work orders. Documents in the clinical record according to requirements of the regulatory agencies. Responsible for oversight of closing out Epic charts.
