Key Responsibilities Patient-Centered Care: Conduct comprehensive psychiatric evaluations, history of present illness (HPI) assessments, and design personalized treatment plans for individuals navigating recovery and mental health disorders. Progressive Monitoring: Document and track patient milestones, adjusting care plans seamlessly in collaboration with clinical leadership and clinic directors. Empowerment & Education: Counsel patients on their treatment journey, helping them understand their progress, and provide essential health education regarding medication management and compliance. Advocacy: Act as a compassionate guide, addressing the physical, emotional, and mental health aspects of substance use and behavioral health disorders to help patients achieve long-term stability and an improved quality of life. What We Are Looking For At least 6 months of MAT / Addiction Medicine treatment experience. Active Board Certification (ANCC or AANP); Addiction Medicine certification or training preferred. Drug rehabilitation training and clinical knowledge preferred . Active, unrestricted Nurse Practitioner license in the State of Tennessee (TN). Satisfactory drug screen and criminal background check. Qualifications: Licensed Registered Nurse in the state where HomeCare agency is located. Minimum one (1) year experience as a RN/professional nurse. Experience in home care is desirable. Position Highlights: Utilizes the nursing process to identify and achieve patient goals: assessment, planning, implementation, and evaluation.
