Function as a case manager for a group of patients in a particular geographic location. Provide after hours care on a rotation basis. Develop and revise an individualized Plan Of Care in partnership with patient/caregiver based on Identification of patient problems/need. Implement specific interventions to attain outcomes. Communicate/collaborate with other disciplines to provide optimal patient care and services. Participate in interdisciplinary team meetings as appropriate. Participate in educational offerings (ex: skills day; oxygen inservice; etc) Supervision and Delegation Appropriately supervise performance of LPN. Must possess valid Illinois drivers license and must be deemed as an acceptable driver in accordance with the MHS Fleet Safety Policy (five year MVR will be required). Must have availability of personal vehicle with proof of insurance. Meets credit range for Clinical Ladder Levels as follows: Clinical Nurse I: Registered Nurse who achieves a range of 0-9 credits on the Clinical Ladder Behavior Grid. Clinical Nurse II: Registered Nurse who achieves a range of 10-19 credits on the Clinical Ladder Behavior Grid with evidence of behaviors from at least two categories of the Grid. Outpatient services include partial hospitalization and intensive outpatient services, as well as The Pavilion Foundation School, a private day school serving students with learning disabilities and behavior disorders. This position will be responsible for assisting residents with activities for daily living. This position does require IDPH CNA certification. To provide outstanding resident care, experience in a geriatric setting is preferred with the demonstrated ability to work effectively with residents and personnel. Major Duties and Responsibilities for Certified Nursing Assistant - CNA: Assist nurses by providing ADL services: Bathing, grooming, dressing, meals, etc. Document vital signs, weight, meal intake, bowel and bladder, etc. Assist with Restorative Nursing duties Answer call lights promptly Performs CNA skills Observes and adheres to all facility safety policies and procedures Identifies safety hazards and emergency situations and helps initiate corrective action Identifies special resident problems, reports them immediately to the nurse, and takes an active role to help problem solve to resolve the issues Assumes accountability of compliance with federal, state, and local regulations within the unit assigned and within his or her work span Reviews resident profiles and perform nursing cares as outlined Able to work independently and is flexible to work all areas when needed Correctly completes required ADL tracking Observe infection control procedures Always leave the resident in a safe environment Ambulates and transfers resident per their transfer policy Helps to mentor new CNAs Restock supplies for the next shift REQUIREMENTS FOR THE CERTIFIED NURSING ASSISTANT – CNA: Current CNA certification in Illinois Active and eligible on the Healthcare Registry Must have reliable transportation to work Ability to work independently Ability to work as a team Maintain an empathetic attitude Possess effective communication skills Compassion for the elderly We offer competitive wages and holiday pay.
