Job Requirements Compensation $30.00 - $38.00/hr Educational / License Requirements High-school diploma / GED Active, unrestricted Licensed Practical Nurse (LPN) / Licensed Vocational Nurse (LVN) license, with residency, in the state of service. Minimum of two (2) years as an LPN/LVN in a direct, patient-facing care-delivery role. Current BLS / CPR certification for healthcare providers, or willingness to obtain. Demonstrated ability to perform high-quality venipuncture and capillary draws on geriatric patients in home settings (where required by market and consistent with state law). Valid driver’s license with an acceptable driving record, and proof of an active auto-insurance policy. Prior telehealth experience preferred. Previous experience in home care, hospice, palliative care, or geriatrics strongly preferred; other complex patient-health-care experience also beneficial. Required Skills and Abilities Strong communication skills, with the ability to effectively interact with patients, families, and remote healthcare professionals. Ability to work independently and make sound decisions while adhering to established protocols, guidelines, and the LPN/LVN scope of practice. Excellent organizational and time-management skills to balance in-person visits, telehealth facilitation, and administrative tasks. Familiarity with electronic medical records (EMR) systems and strong computer skills, including Word, Excel, and PowerPoint. Understanding of risk assessment, quality measures, and full-risk / value-based care. High level of professionalism and a strong geriatric customer-service focus. Experience working with culturally diverse and underserved populations. Physical and Mental Requirements - Ability to stand/sit for extended periods. - Visual acuity and fine motor skills. - Ability to travel to patient homes and facilities as needed. This role is field based Work Environment Field-based role. Daily local travel required, up to 100% locally; visits patients across care settings (home, hospital, SNF, assisted living).
