How you make a difference The Nurse Practitioner (NP) of Medication-Assisted Treatment (MAT) is responsible for providing a full range of medical services for patients under the supervision of the Site Medical Director and other site Physicians. They work within established policy and procedure, as well as state guidelines, to deliver high-quality care to patients seeking MAT for substance abuse disorders. The NP also ensures accurate documentation of services and communicates effectively with the Medical Director and site director to maintain coverage of medical services. Key Responsibilities Conducts on-site chronic care clinics and sick call as established by Site Medical Director, Health Services Administrator, or designee. Provides emergency treatment on-site and responds appropriately in urgent or emergency situations. Prescribes a planned regimen of total patient care, which is adequate, problem-oriented, and appropriate to the needs of the patient. Supports standards of medical care through adherence to existing policies and procedures. Reports any doubts or questions regarding the lack of appropriate referrals, nursing or medical intervention necessary for the realization of established patient goals to the Medical Director or Health Services Administrator for disposition. Additional Details Full Time, M-Friday 8 hour shifts (0800-1600) You should have strong experience assessing and treating patients with substance use disorders, including opioid use disorder and co-occurring medical or psychiatric conditions. You should be skilled in conducting histories and physical examinations, developing and reviewing treatment plans, prescribing and monitoring medication-assisted treatment, and responding to urgent medical needs. Strong clinical judgment, accurate SOAP-format documentation, patient education, regulatory compliance, and multidisciplinary collaboration are essential. Candidates should also be comfortable working independently, interacting with patients with patience and tact, and recognizing when additional consultation or referral is needed. The position ensures timely service delivery by documenting services in the EHR, tracking release timelines, activating Justice-Involved aid codes, and collaborating with in-reach partners and Medi-Cal eligibility staff. Key Responsibilities Provide discharge planning services for medical, mental health, MAT, and related programs in accordance with contract requirements and policies. Identify release-planning needs, coordinate medications and medical needs upon release, and arrange referrals and appointments with community providers. Facilitate psychoeducational sessions, benefit applications, and resumption of services to support continuity of care. Maintain working relationships with facility staff, parole, mental health, and medical partners while adhering to security regulations. Responsibilities This position is accountable for providing and managing competent nursing care to the neonatal patient that requires moderate to complex assessments, interventions and level of nursing vigilance. The NICU RNII is responsible for working in a team environment using a multidisciplinary approach to providing nursing care adapted to individual patient needs based on the nursing process, which includes assessment, planning implementation and evaluation. The individualized patient care is delivered in a safe, supportive, caring and family centered environment. The RN is also responsible for supervising, coordinating and delegating personnel who participate in the provision of care to ensure that patient needs and expected outcomes are met. Qualifications Minimum of one (1) year current NICU Level III experience is required. Current CA RN License, current certification in BLS for Healthcare Providers (CPR & AED) and current certification in Neonatal Resuscitation Program (NRP) are required. RNC-NIC and/or BSN preferred.
