As the Registered Nurse in Home Health you will provide and direct provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies. You will also coordinate total plan of care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources. Primary Responsibilities Clinical Competence Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to State and federal regulations Provides required supervisory visits Documentation and Care Delivery Provides high-quality clinical services within the scope of practice and infection control standards, in accordance with the plan of care, and in coordination with other health care team members Completes comprehensive assessments (OASIS) including medication reconciliation accurately and timely Documents patient visits per policy and payer requirements, and syncs timely per LHC policy Quality Makes initial and/or comprehensive nursing evaluation visits, ensures patients meet home health eligibility and medical necessity guidelines, determines primary focus of care, develops the plan of care within State guidelines with the physician, and submits accurate documentation Communicates relevant information timely and effectively with appropriate agency staff, including patient care issues, visit assignments, schedule changes, orders, OASIS data sets, coding requests, and coordination with other clinicians Communicates timely and effectively with physicians, patients, and family members to ensure quality care and service excellence Teamwork Takes direction from Clinical Director and Executive Director professionally and completes assigned tasks timely, including required learning Assists in the orientation of new agency personnel and serves as a preceptor to other staff and students Actively participates in survey/survey readiness activities and performance improvement plans, works to reduce unnecessary patient hospitalizations, improve patient safety, and implements processes and best practices to ensure positive patient outcomes Participates in on-call and weekend rotation as needed to meet patient needs Adheres to and participates in the agency's utilization management model Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Required Qualifications Current and unrestricted RN licensure in state of Washington Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation Preferred Qualifications Current CPR Certification or ability to complete within 90 days of hire 1+ years of Home Health experience Ability to work independently Solid communication, writing, and organizational skills Pay Range $50.00 - $63.00 hourly Annual total cash compensation for this role assumes full-time employment (40 weekly hours) at full productivity and generally follows the range above. Total cash compensation includes earnings from per visit point pay and hourly pay and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. This role receives two types of compensation depending on the work being performed. When conducting visits, you will be paid per visit point rate compensation. Your per visit pay will be calculated by multiplying your per visit point rate by the productivity points you accrue for various types of visits. Each type of visit is assigned a certain number of productivity points that is inclusive of "direct" and "indirect" patient care activities. Visits are assigned based on patient and business needs. The number of visits performed each week will vary based on individual productivity targets and the productivity points assigned to the visits performed. You will be paid your hourly rate for certain non-visit activities such as orientation. We comply with all minimum wage laws as applicable. Registered Nurse, Transport Team, 0.9FTE Nights, 6PM-6AM This position will float to the following locations: Main Hospital, North Clinic in Everett, South Clinic in Federal Way. As an integral member of the Transport Team, the Transport RN functions within the Scope of Practice as defined by Washington State, the ANA Scope and Standards for Pediatric Nursing Practice, and adheres to Seattle Children's Hospital policies/procedures and guidelines of care. Responsible for the safety of patients prior to and during transport to medical care facilities. This includes the provision of basic patient care as well as advanced life support measures required to stabilize the critically ill or injured patient, demonstration of proficiency in neonatal and pediatric critical care nursing and knowledge of the adaptations necessary for patient management in the ambulance or aircraft and during transit. The Transport Nurse must also be familiar with ambulance and aircraft operating procedures and safety practices. Assists in meeting Seattle Children's strategic goals by incorporating the initiatives to support the strategic plans into the daily work, i.e. Continuous Improvement and Innovation (CII), ART, etc. The Transport RN serves as a team lead within the mobile transport team and may be required to provide supervision and delegation to other Registered Nurses, Respiratory Therapists, or Emergency Medical Technicians who are members of the mobile transport team. We're looking for nurse practitioners in the Tacoma-South Seattle area to lead longevity days at our partner senior living communities. Each day opens with a short educational talk for residents on healthy aging, using a presentation and speaker notes we provide. You then see the residents who sign up for one-on-one longevity visits. You'll be trained and supported by our Chief Medical Officer, a board-certified geriatrician who designed the program's clinical content. Your first site day is run alongside a member of our team. Schedule Site days are on weekdays. The presentation is the fixed part of the day. The rest depends on how many residents sign up. Our services include general cardiology, cardiac diagnostic imaging tests and procedures, cardiac surgery, arrhythmia management and electrophysiology, interventional cardiology, structural heart services, advanced heart failure management, heart transplant and ventricular assist devices (VADs), adult congenital heart disease management, cardiac psychiatry, cardiovascular genetics and other advanced services. POSITION HIGHLIGHTS Conveniently located near public transportation Make a difference for the patients and employees we serve Opportunity to provide career growth Primary Job Responsibilities Provide direct care to patients being admitted and recovered for procedures in the CARDIAC CATH and EP Labs IV therapy, pre-procedure assessment and preparation, post-procedure recovery, access site management, and pre-post patient/family teaching prior to discharge or transfer to the inpatient Cardiology Services Support cardioversions, transesophageal echocardiograms, implantable loop recorders and moderate sedation procedures Requirements Current license to practice as a registered nurse in the State of Washington At least 2 years of cardiac, telemetry, or Interventional radiology experience Current BLS & ACLS Equivalent education and/or experience may substitute for minimum qualifications except when there are legal requirements, such as a license, certification, and/or registration About Uw Medical Center-montlake UW Medical Center is an acute care academic medical center located in Seattle with two campuses: Montlake and Northwest.
