Job Description The Nemours Cardiac Center of the Children’s Hospital in Wilmington, Delaware is looking for a qualified Acute Care Pediatric Nurse Practitioner or Physician Assistant to join our Inpatient Cardiac Center! $30,000 Sign-on Bonus and Relocation Bonus are being offered!! The Nemours Cardiac Center (NCC) provides comprehensive care for infants, children, and adolescents with cardiac, thoracic, and vascular disease. The Cardiac Center provides patient care, performs research, educates and trains health care professionals in a variety of settings. This individual will work collaboratively with physicians, nurses, and other health care providers, dedicating his/her specialized practice to cardiac patients located in either the cardiac intensive care unit, on our telemetry floor or in our post anesthesia cardiac unit (cardiac PACU). The APRN or PA will also be involved with cardiac babies delivered on site in our advanced delivery unit. Our patient population ranges from newborns to young adults with congenital or acquired heart disease. Our surgeons perform all of the complex cardiac repairs, including definitive neonatal surgery, adult congenital surgery, and heart transplantation. They utilize all forms of mechanical support (VAD’s, ECMO, ECLS) as an adjunct to our very active heart failure and pulmonary hypertension service. This position will include working some night, weekend, and holiday shifts. Essential Responsibilities Obtain medical histories, perform physical exams, order and interpret laboratory data, and prescribe management and medications on patients admitted to our inpatient service. Present patients on rounds, write daily progress notes, collate inpatient records, and complete discharge summaries using the electronic medical record. Assist in the preoperative and postoperative care of surgical patients. Provide continuity of care when coordinating care with other members of the health care team. We are looking to add to our Visiting Angels Team! PRINCIPAL ACCOUNTABILITIES : · Understand and adhere to information and precautions contained in individual client’s Care Plan. · Assist the client with personal care activities including: bathing, skin care, back rub, hair care, nail care, dressing and undressing, feeding, oral hygiene, shaving, grooming and bedtime care. · Assist the client to the bathroom or in use of urinal or bedpan. Keep incontinent clients clean and dry. · Assist the client with client's self-administration of medications. · Assist the client with personal communication skills, as needed. · Prepare appropriate documentation of the client care or service(s) furnished. · Report all client and employee incidents/accidents to supervisor immediately. · Assist the client with supervising exercise, ambulation and transfer activities. Be aware of proper body mechanics. · Ensure maintenance of a clean, safe, comfortable and healthy environment for the client. · Observe safety precautions, including: wiping up wet floors, spills and other falling hazards immediately. Also, report safety hazards, frayed electrical cords, unsecured handrails, malfunctioning smoke and carbon monoxide detectors/alarms, defective equipment or environmental hazards to supervisor on the same day of observation. · Engage in respectful social interaction with client, including friendly conversation and empathic support, as well as, respecting client’s privacy and property. · Exhibit positive attitude and behavior and demonstrate respect for employees and clients and maintain absolute confidentiality of all information pertaining to clients, clients' families and employees. · Respond and attend to client’s requests promptly. · Communicate effectively with all members of the team. · Maintain proper hand washing techniques. · Participate in required in-service training programs to meet regulatory requirements. · Understand the importance of seeking assistance, as needed, from your supervisor and/or others; demonstrate capability and dependability in following instructions. · Understand that regular, consistent attendance is necessary to serve clients. · Accept and fulfill assignments with the Agency; exercise judgment in accepting assignments. · Perform related duties and responsibilities as deemed appropriate by the management team. SPECIFIC JOB KNOWLEDGE, SKILLS AND PERSONAL ABILITIES REQIRED : · Must be eighteen (18) years of age. · Demonstrate sympathetic attitude toward caring for others. · Previous experience in private duty home care preferred. · Ability to listen and communicate clearly, fluently and diplomatically orally and in writing. · Ability to remain flexible, resilient, calm and maintain a sense of humor; and present a well-groomed professional image. · Ability to generate goodwill for the organization and its management among staff, clients, and referral sources. The Opportunity The Caregiver position provides support for our community’s residents with the essential day-to-day tasks they can no longer perform or need help with. Caregivers are responsible for helping people with their daily tasks, such as laundry, cleaning, bathing, dressing, and taking their medication. Successful Caregivers are sensitive, respectful and are passionate about helping those in need. What You’ll Do Helping with personal hygiene, including bathing, grooming, and dressing Supporting residents with medication Repositioning residents in bed and escorting them around the community Toileting, incontinent care Take vital signs, such as blood pressure, heart rate and pulse Observe and report on residents’ health, escalating any problems efficiently and effectively Detail daily reports to Nurse, or designated charge person, at end of shift Provide emotional support and serve as a source of comfort for residents and their loved ones Assist residents with mobility by helping them get in and out of bed, chairs, or wheelchairs.
