Supports the District Director of Clinical Services (DDCS) in managing the healthcare operations of the communities in an assigned district. Helps ensure residents’ healthcare needs are met in compliance with state and federal regulations as well as Brookdale standards. Travels within the district as assigned to support management vacancy, training, auditing and any other area identified to ensure clinical excellence and meet state requirements. Travels throughout the district weekly to communities as assigned by the DDCS. Fills the role of HWD in assigned communities when an HWD is on leave, extended vacation or the position is vacant. Supports clinical initiatives and roll-outs by leading training for the district and/or individual communities as assigned by DDCS. In some states coordinates and signs resident assessments. This may include completing comprehensive assessments including personal interview with the resident and other if the resident due to any impairment needs assist with history, etc. Facilitates RN Delegations for medications and care per state regulations. Participates in a variety of meetings with the division, district and individual communities. Provides training, coaching and guidance to clinical leaders and front line staff. Participates in clinical training and audits compliance; including facilitating Med Tech/QMAP/Med. Aide training and competency evaluations. May assist DDCS in clinical Quality Assurance (QA) site visits. Provides insight to DDCS on overall assessment of community strengths, weaknesses, opportunities, and threats (SWOT). May be asked to provide regular reports. Provides support in clinical investigations and may participate in disciplinary actions. Supports DDCS with special projects as assigned. Location: Henry Wade Juvenile Center PRIMARY PURPOSE Responsible for providing total nursing care for assigned patients, under the guidance of a Registered Nurse, in order to ensure quality care. MINIMUM SPECIFICATIONS Education Must be a graduate of an accredited school of vocational nursing and/or licensed by the Texas Board of Nursing as a Licensed Vocational Nurse; or have a valid Compact LVN License. Experience None. Virtual Care Only: 2 years healthcare experience preferred. 1 year of care coordination and planning, chronic disease management, population health, and/or telehealth preferred. Certification/Registration/Licensure Must have current, valid LVN license or temporary LVN license from the Texas Board of Nursing; or, valid Compact LVN license. Reinforces patient/family education teaching that was initiated by RN or NP. May participate in community outreach activities (such as health fairs, community collaborations or outreach clinics) as assigned. Assists with precepting (training, motivating, and supervising) new employees and students as directed by supervisor. Completes appropriate written and computer documentation according to established procedures and guidelines. Consults with other team members as required and provides nursing care which demonstrates patient centered/patient valued care while anticipating and accommodating patients, families, physicians, and other system staff to ensure the workflow is coordinated and patients receive timely and appropriate treatment. Ensures that appropriate supplies are available for individual patient care by gathering all necessary items together in advance. May assist with ordering and stocking supplies as needed. Responsible for cleaning and sterilizing both patient areas and medical instruments to ensure an appropriate healthcare environment. Identifies ways to improve work processes and improve internal and external customer satisfaction. May assist with developing annual goals and objectives for the nursing area that support the mission and objectives. Makes recommendations to supervisor, implements and monitors results as appropriate in support of the overall goals, objectives and mission of Parkland. Stays abreast of the latest developments, advancements, and trends in the field of nursing by attending seminars/workshops, reading professional journals, and maintaining LVN licensure. Integrates knowledge gained into current work practices. Sets up patient appointments and referrals for further testing and follow-up care as needed. Assists in transporting patients as needed. Virtual Care Only: Actively participates in proactive patient outreach utilizing motivational interviewing approach to coach patients, support health behavior change, and collaboratively set goals toward self-management, self-efficacy, and activation. Instructs the patient on condition management, behavioral support, and addresses emotional demands while reinforcing care plan and follows ups. Conducts a comprehensive chart review with focus on inadvertent diagnostic findings requiring follow up testing and closing gaps in care. Assists patients and families with health system navigation, making appointments, patient advocacy, determination of community resources to reduce barriers to care, and support set-up and initiation of electronic communication modes (email, MyChart, etc.).
