What you'll do The primary purpose of this position is to establish, organize, direct, and implement the Cardiac Rehabilitation Program in accordance with Federal, State, and local guidelines. The Cardiac Rehabilitation Nurse evaluates & treats patients with a wide variety of cardiac disabilities, in an outpatient setting, helping to achieve risk factor reduction in cardiovascular disease using various therapeutic exercises, activities, and educational classes. The Cardiac Rehab Nurse is responsible for documenting all patient care appropriately in the medical record to comply with the policies and procedures of the department and JCAHO requirements. Responsible for orientation and initial assessment of participants in the cardiac rehabilitation program Develops plans of care for new participants Documents initial assessment and daily participant information and progress Leads and/or delegates participant education sessions based on staff expertise and identified participant needs Develop and maintain current educational materials for patient education Assess participants pre-, during, and post- exercise Supervises exercise sessions and/or delegates based on staff experience Obtains monthly required approval of individualized treatment plans from program medical director, per Medicare guidelines Monitors participants for inappropriate or abnormal responses to exercise Makes appropriate referrals and /or recommendations to participants, the participant’s personal physician, and/or program Medical Director as needed or as requested by patients Reports any significant participant conditions (dysrhythmias, abnormal blood pressure response, change in participant’s status, etc.) immediately to the physician supervising the program Initiates ACLS measures and/or emergency care according to facility policy Checks AED equipment and other emergency supplies and equipment for proper function and adequate supply per facility policy Set up and supervise emergency drills and mock codes on a routine basis giving appropriate feedback and constructive criticism where appropriate Optimize reimbursement for cardiac rehabilitation services through effective communication with all cardiac rehabilitation staff, including support and/or office staff Coordinates and/or maintains Continuous Quality Improvement program and participates in or delegates participation in quality reporting program per facility policy Maintain current knowledge in the field of cardiac rehabilitation through resources including but not limited to: cardiac rehabilitation literature, in-services, continuing education courses and/or conferences Responsible for coordination of any routinely occurring meetings, including: organizing and planning guest speakers, education, food, and/or public communications What we need from you Education: Must be a graduate of an accredited school of nursing, required *** All RN’s hired and do not currently hold a BSN will be expected to demonstrate enrollment in a bachelor’s prepared nursing program within 4 years of hire. What you'll do The Registered Nurse - Float Pool will provide nursing care to wide variety of patients within Bassett Healthcare Network (BHN). During every shift, the RN will be a member of a nursing team that provides direct nursing care to a specific unit within the hospital. It is quite conceivable that a RN may start their shift in one unit and then based on a change in census and acuity be transferred or moved to a different unit to meet operational requirements. However, it is possible that an RN may spend extended periods of time in one nursing unit to help balance off long-term absences (i.e. illness, holidays, maternity leave, etc.). While working within a nursing unit the RN acts as a patient advocate and facilitates communication between the patient, family and other health care professionals. This will result in a holistic approach to planning, organizing, teaching and relationship development that will best meet the needs of the patient. Based on patient census and acuity throughout BHN the incumbent may be required to provide standardized nursing care to any and/or all of the nursing units within the scope of a RNs practice. This scope includes medical-surgical, telemetry and acute care (i.e. emergency department). What you'll do The LPN Clinic Coordinator provides direct and indirect patient care under the direction of the Registered Professional Nurse or Physician. The LPN functions as part of the interdisciplinary health care team. The LPN Clinic Coordinator provides direction for daily nursing assignments and acts as the liaison between the medical staff and the nursing staff regarding clinical issues. Works with the Nurse Team Leader, Supervisor, and/or Manager to assure the appropriate application of the nursing process Assists in the orientation, training and documentation of new clinical staff according to random sampling of personnel records by supervisor Works directly with the team leader, supervisor and or manager to assess and meet continuing education needs and requirements for self and nursing staff, as identified through observation Functions in a staff nurse capacity at least 50% - 75% of time and as needed to ensure quality patient care and safety, as observed by supervisor Works directly with team leader, supervisor, and the manager to implement Performance Improvement and Risk management activities and practices, as observed by supervisor or manager As delegated, makes clinical assignments, monitors performance, sets work schedules and arranges coverage Post schedules at least four to six weeks in advance, as observed by manager/supervisor/team leader Monitors workload, shifts staff as needs, as observed by manager/supervisor/team leader Independently solves staffing problems, keeps manager/supervisor/team leader informed of situations beyond his/her resources, as observed by manager/supervisor Provides input for evaluations of LPN’s in clinic as requested on time with 100% compliance, as reviewed by manager/supervisor/team leader Assists with interviewing candidates for open nursing positions giving useful feedback to the supervisor/manager/team leader in requested timeframe Assists in the development of proposals for improved efficiency, productivity or expansion of nursing services, offers workable suggestions with follow through, as observed by team leader/supervisor/ manager Monitors time and attendance of LPN’s in clinic and addresses issues as they arise in conjunction with team leader/ supervisor/ manager Assists in the development of effective interdepartmental working relationships, to ensure smooth, efficient patient care systems Sets an example in work performance and team building attitudes, concern for patients and support of clinicians by maintaining a positive interpersonal attitude and demeanor, as observed by manager/supervisor/team leader Facilitates meetings/huddles with staff, during provider educational time, to discuss patient and clinic concerns.
